Failed rhinoplasty: from 5 to 15% of patients decide to have a second operation (facts)

With rhinoplasty, as with any other operation, complications are possible. Sometimes complications are so severe that they affect the outcome of the operation and are difficult to eliminate. In this case, rhinoplasty is considered unsuccessful.

It also happens that the result of rhinoplasty does not coincide with the patient’s expectations. This does not always mean that the operation was unsuccessful. It happens that the anatomical features of the nose simply do not allow one to achieve the ideal that was formed in the patient’s imagination. The surgeon always strives to warn about this in advance and discusses with the patient possible options and the expected final result of plastic surgery, including using computer modeling.

Tilda Swinton: The White Witch

The White Witch in the film “The Chronicles of Narnia: The Lion, the Witch and the Wardrobe” looks more like the Snow Queen. Her unearthly appearance harmonizes perfectly with the image of the villain and at first glance seems attractive. This is what allowed her to charm young Edmund Pevensie. Stylists created the perfect dress for Tilda, creating an imitation of fabric woven from snowflakes, and the crown seemed to be made of real icicles. Gradually, as the plot develops, the queen loses power over Narnia, winter is replaced by spring, and her outfit also transforms: from snow-white it becomes gray, and the ice crown melts.

Marina Alexandrova: Catherine II

Marina Alexandrova reincarnated as Empress Catherine the Great in 2014. She was well able to show the young and timid Princess Sophia to Augusta Frederick, who had to do everything according to the orders of Empress Elizabeth. It was only later that her character became stronger and her ambitions emerged. For filming in the second season, where Alexandrova’s character took the throne, she had to gain 10 kilograms to look more convincing.

Kirsten Dunst: Marie Antoinette

The Queen of France and wife of Louis XVI is shown in Sofia Coppola's film “Marie Antoinette” in the most grotesque image.
It is known that the queen loved lavish balls, dresses and spent money from the treasury on her entertainment. Kirsten Dunst appeared in the film as a frivolous lady, wearing dresses that looked like marshmallows. All of them were made in soft pastel colors, with bows and ruffles. She had high hairstyles on her head, typical of the 18th century, and sweets that the spoiled daughter of the Austrian emperor loved so much were often glimpsed in the frame. The film did not receive very high marks from critics due to the fact that it does not contain enough historical facts, but Kirsten Dunst is a pleasure to watch.

Monica Bellucci: Mirror Queen

Source: @dark_romeo_92
In the film "The Brothers Grimm" Monica Bellucci played a negative character, but how good she is in it!
Some viewers probably regretted that she was killed off at the end. In the fantasy film, she showed her charm and grace in all its glory. Stylists created for her a scarlet and gold dress with a deep neckline and an unusual headdress that looked like a crown. Her luxurious long hair is also striking in the film. To create such volume and length, make-up artists used a wig. However, the actress has luxurious curls even without wigs.

The real story of facial plastic surgery with surgeon Andrey Iskornev

On the eve of her anniversary (50 years), our patient Marina really wanted to perk up, get new emotions, look younger and love herself again!

Photograph of the patient 3 days before surgery.

It was difficult to decide on the operation, even though Marina dreamed about it so much. The excitement went away as soon as Marina talked with the doctors and anesthesiologist, recharged with energy, gained a positive attitude and confidently lay down on the operating table.

First day after surgery.

The doctors remember how Marina was in the right mood during the post-operative period: she always said: “Everything will be fine with me, all the bruises will go away and I will be the most beautiful!” And so it happened!

Marina shines at the party. A little over a month has passed since the operation.

Charlize Theron: Ravenna

Source: @tory_numero
In 2012, Charlize played the role of the queen in the film Snow White and the Huntsman. For her look, 20 outfits were created that demonstrated her domineering character. Each dress took weeks of labor to create, but some appeared in the frame for just a moment. The most memorable was the black outfit in which she turned into a flock of crows.

The actress said in an interview that she liked playing the villain. She got into the role so much that she tore her abdominal muscle from screaming on the set (and this is not a joke).

Reasons for unsuccessful rhinoplasty operations

Although such a complex operation as rhinoplasty has been carried out for many years, no specialist can guarantee an absolute result. What are the reasons for this:

  1. A plastic surgeon cannot accurately predict the outcome of rhinoplasty because each body behaves individually. Everyone can heal differently: blood vessels, nerves, nasal bones, cartilage, mucous membranes;
  2. The result after surgery may change after a year.
  3. Rehabilitation after rhinoplasty is also individual, so you need to be under the supervision of your doctor at all times.

The first reaction after a failed operation is to blame the doctor. But we recommend that you figure out what the reason is: a doctor’s mistake or the body’s reaction. A plastic surgeon with extensive experience in performing rhinoplasty must certainly notice the complications that arise and respond to them in a timely manner.

Doctor's mistakes

  1. During rhinoplasty of the nasal hump, if the surgeon performs the operation incorrectly, a depression may appear on the nose;
  2. During surgery, skin detachment may occur, which can lead to tissue necrosis;
  3. If the doctor removes too much cartilage or bone tissue from the nose, the resulting organ will look unnatural;
  4. During the operation, the doctor changed the size of the wings of the nose. This is a dangerous complication of nasal breathing. When inhaling, the nostrils will stick together;
  5. If the doctor took the measurements incorrectly before rhinoplasty, you may end up with an asymmetrical nose after the operation.

Cate Blanchett: Elizabeth I

Source: @cate_blanchettofficial
“I am your queen,” Kate says at the end of The Golden Age. Indeed, her aristocratic appearance and porcelain skin made it possible to fit perfectly into the image of Elizabeth I. For this role she was nominated for an Oscar and a Golden Globe.

In the film, Blanchett appeared not only in luxurious dresses in the halls of the palace, the film also included scenes from the battlefield, and the queen dressed in armor. Kate admitted that the armor was real and she had a hard time wearing it. But it was even more difficult in Elizabeth I’s outfit for the coronation ceremony. For him, the costume designers built a rigid corset attached to a wooden base. In this dress, the actress had to keep her back straight and it was quite difficult to move.

Julia Roberts: Queen Clementianne

In 2012, another version of Snow White was released, in which Lily Collins played the main character and Julia Roberts played the stepmother. In this film, the negative female character turned out to be more charming than Princess Snow White. The change in images of Julia Roberts' character is pleasing to the eye. Her spectacular outfits and unusual hairstyles are a real work of art. It's hard to even pick one.

Margot Robbie: Elizabeth I

Margot Robbie also played Elizabeth I.
In an interview, she admitted that it was an interesting experience - she was tired of the roles of frivolous beauties. In the 2021 film “Two Queens,” Margot got a dramatic role. If at the beginning of the film Elizabeth I looks majestic, then towards the end of the plot, in order to show the consequences of the illness, the actress was made up. The image turned out to be repulsive, but memorable.

Functional complications

In case of functional complications, treatment should be started urgently. Aesthetics fade into the background: your current priority is maintaining your health. Consult your doctor immediately if you experience the following after rhinoplasty:

  • Intense nosebleeds;
  • Severe and persistent inflammation;
  • Severe pain in the soft tissues of the nose;
  • Discharge of pus or ichor from the nostrils;
  • Modification of the skin (darkening, blue discoloration, etc.).

According to statistics, only 25-30% of all plastic surgeons masterfully operate the nose. Meeting an experienced rhinoplastic surgeon is a real happiness for a person who dreams of a beautiful nose. Be prudent, careful and responsible when choosing a surgeon - this will help protect you from negative consequences associated with the operation.

Emily Blunt: Queen Victoria

Source: @di_bedzhanyan
In 2008, Emily Blunt played the role of the young Queen Victoria, who is just ascending the throne and gaining the trust of her people. Many critics noted that the actress does not look like the prototype - she is too good for him, but the dresses were appreciated. The costume designers recreated the outfits as closely as possible to the original ones. Each of the heroine's dresses was insured for £10,000.

How to behave before and after rhinoplasty

Violation of the doctor's instructions when preparing for surgery, and improper behavior after, can reduce all the surgeon's efforts to zero. In the process of preparing for rhinoplasty, you must:

  • completely give up bad habits, such as alcohol and smoking, they can provoke poor tissue healing;
  • It is forbidden to take drugs that affect blood clotting in order to avoid bleeding after surgery;
  • after rhinoplasty, you should not visit the solarium or sauna in order to avoid surges in blood pressure;
  • Any physical activity is prohibited, especially those that require bowing your head;
  • It is recommended to sleep on a high pillow or with the head of the bed raised. This will help reduce swelling and avoid nasal deformation;
  • within a month after rhinoplasty, it is not recommended to consume too cold or hot drinks and dishes;
  • for 2 weeks you need to wear a special plaster splint, which serves as a frame for the nose during the healing period;
  • It is forbidden to wear glasses during the rehabilitation period, they can deform the tissue. If necessary, you should use contact lenses;
  • You should not plan a pregnancy for a year after rhinoplasty;
  • Avoid blowing your nose for 2 months after surgery. To cleanse your nose, you should use sprays prescribed by your doctor. Vaseline or peach oil will also help to moisturize.

Before rhinoplasty, it is very important to choose an experienced doctor, a highly specialized clinic, and read real patient reviews. However, responsibility is not removed from the patient either: it is important to provide honest information about your health, pass all the necessary tests, and follow doctors’ recommendations. But, even if all these conditions are met, the risk of negative consequences cannot be excluded.

Lena Headey: Queen Cersei Lannister

Source: @iamladymargaery
The golden-haired and powerful queen from the series “Game of Thrones” gradually loses control over her emotions from season to season and it seems that she is about to lose her mind. She will stop at nothing to achieve her goal. There are moments in the series in which Queen Cersei walks down the street naked for the sake of atonement, but a stunt double was filmed in these scenes.

By the way, while eight seasons of the series were being filmed, Lena Headey managed to become a mother twice. However, dresses with long, wide sleeves made it possible to hide the belly. You may notice that her character often crosses her arms over her stomach, disguising her pregnancy.

Another story of facial plastic surgery of patient Iskorneva A.A.

My name is Irina, I am 43 years old.

I started thinking about facial plastic surgery at the age of forty, when I discovered real signs of fading. Over the course of three years, despite my efforts, these signs bloomed more and more magnificently and felt more and more confident on my face. The same cannot be said about me. My confidence diminished greatly, as did the joyful carelessness in which I had lived almost my entire life. And so, when “the waters reached the soul,” as if by magic, they announced another competition on the Plastic for woman website together with plastic surgeon Andrei Alexandrovich Iskornev. There was no trace of doubt (to do or not to do).

Iskornev A.A.:

Irina is at an age when the surgeon faces a dilemma about how radical a method of rejuvenation to choose. Since Ira wanted to get a radical result of lifting the contour of the lower jaw, getting rid of bags in the lower eyelid area, and tightening her eyebrows a little, I decided to choose a combination - facelift (circular facelift with SMAS strengthening), endoscopic brow lift in the temporal region, transconjunctival blepharoplasty of the lower eyelids ( remove bags) and lipofilling (the finishing touch to fill the volume of the face and improve the quality of the skin). The scope of the operation is quite large, meaning that the patient has time for rehabilitation. Otherwise, I would have chosen a combination of thread lifting methods - Silhouette lifting or Intertemporal lifting, but the result of the operation would have been shorter.

Surgeon's recommendations

1 day before surgery

Try to limit yourself to a light dinner at 19-00, after which you do not take any food or drink. Take an antibiotic recommended by your doctor as a preventive measure.

On the day of surgery

Arrive at the clinic 3-4 hours before surgery strictly on an empty stomach. You will be changed into disposable underwear, photographed and given access to medical documents. After this, the anesthesiologist makes premedication, and the surgeon makes preoperative markings.

Diet after surgery

You will be advised to avoid eating fried, spicy, and salty foods. For the first 2-3 days, it is better to limit yourself to vegetable and pureed foods to minimize chewing movements. From 4 days you can return to your normal diet.

What to buy in advance at the pharmacy?

Antibacterial drug recommended by the doctor, Liaton 1000 gel, Traumeel S cream, Maxitrol and Emmoxipin eye drops. Individual appointments can also be given.

When are stitches removed?

Usually, the surgeon removes the fixing sutures in the scalp 8-10 days after surgery, and the intradermal pre-auricular sutures on the 7th day. Within 1-1.5 months they turn white and take on a flesh-colored tint.

2 weeks before surgery, you should stop smoking cigarettes (or reduce them to 2-3 per day), and avoid drinking alcohol.

“I fell asleep and woke up in the ward, but then I still really wanted to sleep”

Irina became the heroine of the project with the largest number of surgical procedures in one anesthesia. Iskornev A.A. performed liposuction of her chin, facelift, transconjunctival blepharoplasty, endoscopic lift of the eyebrows and temporal region and completed everything with lipofilling, that is, filling wrinkles and folds of the face with her own fat to simulate volume and a more natural result of the operation.

Irina:

“The only thing I was worried about was that I would hear what was happening in the operating room. But almost immediately I fell asleep.”

The operation was performed under general anesthesia under the guidance of the most experienced anesthesiologist Natalya Georgievna Sarkisova, Ph.D. with more than 30 years of experience in aesthetic surgery.

“We usually perform operations up to 3 hours under intravenous TIVA anesthesia. When it comes to longer complex interventions, it is preferable to use general anesthesia, when a special Drager device ensures the constancy of respiratory function and guarantees a calm operation,” says Natalya Georgievna.

Progress of the operation

At the very beginning of the operation, the surgeon performs liposuction of the chin. In Irina's case, we used a laser to further reduce the skin. Then transconjunctival blepharoplasty was performed, which removed the fat bags of the lower eyelids without making a skin incision. Next came the longest stage - facelift (tightening the skin of the face and neck with a pre-auricular incision). With this operation, it is not so much the skin that is tightened, but the internal structures, which are united by the concept of SMAS. Since during such an operation excess skin is formed in the temporal region, Iskornev A.A. Then he performed an endoscopic tightening of the skin of the temple and at the same time lifting the “tails” of the eyebrows. The only incision from such an operation is located in the scalp and is completely invisible in the future. And the finishing touch was lipofilling, during which Irina was given additional volume in the cheekbone area and the so-called tear trough. All this together ensures maximum naturalness as a result of the operation.

This is what the patient’s “inner world” looks like through the surgeon’s endoscope, which is used to lift the temporal area and eyebrows through a small incision in the temporal region. Magnification on the monitor helps the doctor avoid the slightest inaccuracy in performing the operation.

Blood tests before facelift:

  • General blood analysis
  • Blood chemistry
  • Coagulogram (blood test for clotting)
  • Testing for HIV, hepatitis C, hepatitis B, syphilis
  • General urine analysis
  • ECG
  • Examination by a gynecologist (if indicated)
  • Consultation with a therapist (if indicated)

Joyful changes

Irina on the 10th day after surgery, isolated bruises on the soft tissues and sclera of the eye. It has no effect on vision!

Irina:

“I’m starting to notice very happy changes. The swelling decreases in the eyes, the numbness in the ear area begins to gradually disappear, the hemorrhage on the left eye resolves, just like an ordinary bruise. Most of all before the operation I was worried about my oval face - how to remove jowls. The pictures show that in these places everything has become stronger and tightened, the ends of the eyebrows have risen, which I really like! At the same time, they are not too raised, as I asked the doctor. I don’t want to look constantly surprised.”

Iskornev A.A.:

“Irina has excellent dynamics. Considering the extent of her surgery, she is recovering very well. Despite the bruises on the left side of the face, it is clear that the swelling is beginning to go away. The drops and physical therapy prescribed to Irina will help Irina speed up this process. The contours of the face are symmetrical, motor function is not impaired. Plus, it’s noticeable that her spirits are very high.”

14th day after surgery. Coming back to life

Irina:

“In the morning, looking at myself in the mirror, I was pleased to discover that the condition of my eye (sorry for the simple tautology) was improving before my eyes.:) Immediately, daring plans and desires began to arise in my head. for example, going to the hairdresser or shopping.”

The patient before and 2 weeks after a circular facelift.

This is what the suture looks like in the pre-auricular area on the 20th day after surgery.

Additional procedures that enhance the results of the operation

Bio-reinforcement of the face RADIESSE

The technique enhances the results of a surgical facelift, enhances skin collagen production, improves skin tone and can be performed 3 weeks after surgery. The effect of Radiesse bio-reinforcement will increase every day!

Fractional laser resurfacing of the face, neck, décolleté

If a plastic surgeon works more with the lines of your face, then dermatocosmetologists care about the quality of your skin. Laser fractional resurfacing or a course of peeling will help remove enlarged pores, improve skin color, straighten fine wrinkles in the eye area, and make the skin more elastic. The technique is ideally combined with surgical types of rejuvenation both before and after surgery.
