KIRILL PSHENISNOV: “We have plastic surgery, not a marketing strategy”

The famous plastic surgeon, Doctor of Medical Sciences, Professor Kirill Pshenisnov heads the EMC Plastic Surgery Clinic, located on Orlovsky Lane. He has more than thirty years of practical experience in the field of reconstructive surgery. Dr. Pshenisnov is fluent in techniques for restoring harmonious facial proportions:

Doctor of Medical Sciences Pshenisnov heads the plastic surgery course at the Yaroslavl State Medical Academy, works as a professor in the department of plastic surgery at the Russian State Medical University in Moscow. He is the first Russian surgeon to complete the full training program of the Plastic Surgery Foundation in the United States. For several years he trained in the field of aesthetic, reconstructive and plastic surgery in leading European and American clinics.

Professor Pshenisnov was repeatedly invited to give lectures and reports, perform demonstration operations in the United States of America, the Netherlands, France, Germany, Italy, Finland, Australia and other countries. He is a member and founder of the Russian and international societies of reconstructive and plastic surgeons, heads the board of the Northeast Non-Profit Partnership of Plastic and Reconstructive Surgeons, and is a Corresponding Member of the US Society of Plastic Surgeons. KIRILL PSHENISNOV: “We have plastic surgery, not a marketing strategy”

Plastic surgery does not stand still. New techniques are being introduced, capabilities are expanding, and trauma is being reduced. For many, going to surgery is equivalent to going to a restaurant. Everything becomes simpler and more accessible. Our interlocutor adheres to an academic approach and is sure that there is no place for frivolity here.

Kirill Pavlovich, our last big conversation for the magazine took place 4 years ago. What has changed since then in your life? A lot has changed in plastic surgery, in the country, in the city. One of the most important events is that in 2009, plastic surgery was recognized as an independent surgical specialty, the same as traumatology, orthopedics, urology, obstetrics, and gynecology. Since January of this year, we have resumed training programs in plastic surgery in Yaroslavl. In September we plan to accept residents for training not in short-term courses, but for 2 years. And we will be responsible for ensuring that they become real specialists. For the previous 3 years, I gave all my lectures in this specialty in Moscow, at the 2nd Medical Institute (RNIMU named after N. I. Pirogov). By the way, most of my practice now takes place in the capital, in conditions that are not available even in many clinics in Europe.

Four years ago you talked about plans to create your own clinic in Yaroslavl. What has been done in this direction? As a surgeon, I need working conditions that are appropriate to the operations being performed. I dreamed that investors would be interested in the opportunity to create a European-level multidisciplinary clinic in our city, where I could operate. So that every patient, entering the walls of this medical institution, sees and feels that everything here is done at the level of the best world standards. Western millionaires, such as Carnegie, Rockefeller and others, have invested and continue to invest in universities and medicine. And the best domestic businessmen prefer to spend money on football clubs. Unfortunately, little has changed in this regard in 4 years. In European and American healthcare systems, the patient comes first, then the doctor, and only then management. With us, everything is exactly the opposite: leaders play the first roles. And I really want to work where the European approach is applied to the patient. Therefore, I was forced to create my own Plastic Surgery Center. But I'm not a businessman - I'm a professional surgeon. And when there are interesting offers from good managers, I accept them. Therefore, I currently practice at the European Medical Center in Moscow.

In 2010, the textbook “Course of Plastic Surgery”, of which you became the author, was published. Yes, this is a lot of work and a joyful event for me! But it came as an absolute surprise when the craftsmen immediately made pirated copies of the textbook and posted them on the Internet. We hoped that at least the costs of publishing the book would be recouped. All this speaks about the general level of culture in our country and the conventions of copyright. In America, for example, it is simply indecent to make a copy of more than 10% of a publication, because this violates the rights of the author. And I can only be happy for those who received a good textbook easily and free of charge. Today it is the only one of its kind.

Kirill Pavlovich, you don’t need to be a specialist to notice: the number of plastic surgeons has increased in recent years, if not significantly, then by an order of magnitude. The problem in our country is the boom in newly minted plastic surgeons. There was a desire to do everything at a high level, so that people would not just listen to a course of lectures, but would answer test questions and show what they could do with their hands. However, many felt that it was enough for them to take paid 5-month courses, while skipping most of the classes. I know a number of universities that issued diplomas without even having a license. As a result, patients suffer, because not everyone knows what the price of that diploma that hangs in the surgeon’s office is.

There is an interesting proposal - to adopt the European system of training doctors, when points are awarded for attending international conferences, where there are test questions that can be used to test knowledge. For more than 50 years, independent expert boards have existed in America and Europe that enable doctors to take such exams. These requirements are not mandatory, but only by fulfilling them will you be recognized as a professional. In other words, in our country we have not yet learned to regulate the process and quality of training of medical specialists.

You talk with restraint, but you are probably overwhelmed with emotions when you hear about the actions of the newly minted doctors. Emotions aside. There is a struggle of ideas. Not long ago, on the Dozhd TV channel, Ksenia Sobchak and Artemy Troitsky discussed what the Russian people need. The presenter is sure that people want simple things - songs by Nikolai Baskov and books by Daria Dontsova. This is the level of development. Troitsky, on the contrary, argued that people should strive to read serious works and listen to interesting and high-quality music. In our profession, there is also a struggle between two ideas: selling a service like a meal at McDonald's, supported by good marketing. Or stick, as I do, to an individual, academic approach.

I am glad that many patients take plastic surgery seriously and I enjoy working for them. In the team where I work, the emphasis is always on professionalism and attentive attitude towards everyone. We have plastic surgery, not a marketing strategy.

Interesting observation. I recently met with Americans who are conducting research on the Moscow market. They counted 250 clinics, but half of them don’t even have websites or their own premises. The choice is very wide both in the capital and in other cities.

You took part in the show “Terribly Beautiful” on one of the Russian TV channels. What were your impressions of working under television cameras? Plastic surgeons around the world are very attentive to such shows. Program participants receive surgery free of charge, but it is important that they receive the same treatment as they would during a commercial operation. You can’t just take a volunteer off the street, operate on him and refuse to observe him.

My patient was from the Moscow region, we performed breast and nose surgery at the same time, and observed her throughout the prescribed rehabilitation period. I agreed to participate because I knew that the operation would take place in a very good clinic, and I would have the opportunity to observe my patient. In addition, I understand very well the feelings of people who decide to talk about their shortcomings and ugliness into the lens of a television camera.

Didn't the project turn into a show? When I joined the project, the seventh season of “Terribly Beautiful” was already in progress, and the entire film crew worked absolutely professionally. There were no moments that were offensive to the patient; nothing unnecessary was shown that should remain within the profession of a plastic surgeon. Everything is calm, without sensations and excitement about the “birth of another person.” There are 7 thousand plastic surgeons in America who have studied their specialty for many years. But there are also from 20 to 100 thousand doctors who perform plastic surgery, being ENT doctors or dermatologists. Professional plastic surgeons cannot ban their practice, but they can show the benefits of fully trained and certified specialists. When participating in a TV show, real specialists carry with them professional ethics and do not allow a serious operation to be turned into a stupid show.

Has patient attitudes towards plastic surgery changed in recent years? Competition has increased, and patients increasingly come to us after visiting a number of other clinics. It happens that people consult with one surgeon, just in case they go to another and hear diametrically opposed judgments. Over these 4 years, life within the Russian community of plastic surgeons has not become more eventful or productive. Quite the contrary. One of my patients decided to get additional consultation from Moscow surgeons after the operation. In one place they told her that they could give her a 20 percent discount. Another clinic offered to redo it again. And they didn’t even ask who performed the operation and how. True, after patients voice my last name, colleagues usually take their words back.

It is difficult for patients to choose, tension has increased, but they are gradually learning to find good specialists. And I only welcome it if my clients consult several clinics in order to compare and make the only right decision.

Probably, the attitude towards the rehabilitation period has also changed. After all, you need to be observed for a year, but the operation itself no longer reminds you of itself... Recently I had a patient from Ukraine, we performed reconstruction of his auricle. He was in a hurry and insisted that they use a silicone implant rather than costal cartilage. As soon as everything healed, he left, abandoning the rehabilitation period. I did the dressing myself, but applied the bandage too tightly, resulting in complications and a repeat operation. Not following the prescribed postoperative regimen, not showing up at the appointed time... Many patients believe that everything is simple: do it and forget it.

If we take a purely legal approach, then you have violated the contract, which means we refuse to monitor. I bought a car and didn’t undergo maintenance on time, which means I lost the warranty. But we are doctors, and our duty is to do everything to ensure that the treatment is completed successfully. The doctor is always on the patient's side, but there are obligations on both sides. We recently talked with a girl from Vologda: she had an operation with us, but had an accident and was unable to come for a scheduled examination. In such situations, we always find a compromise solution.

I can tell you an interesting story that happened to a Moscow businesswoman. She had a face and neck lift, and she decided to complete the journey that lasts a month in a week. I learned something from the Internet, and after lifting and complete detachment of the facial skin, I began to massage it. When the skin turned blue, she began to rub onions into her cheeks. It didn’t help - I decided to apply an iodine net and warm it with sea salt. In the end, I burned it all with vinegar essence and came to the appointment with chemical burns on both cheeks. Until the last moment, she was sure that she did everything correctly, because that’s what she was advised on the Internet. This is no longer frivolity, but psychological problems.

Do you often refuse patients? In fact, it would be worth doing this more often. The surgeon sees a problem such as a hump on the nose or a protruding ear, or a problem with the skin. Technically we can fix everything, but psychologically it’s more difficult. First, the administrator talks to the client, then the manager, they find out how realistic the requests of the possible patient are. And the most important thing is whether he understands the seriousness of the upcoming step, whether he can follow the doctor’s recommendations and regimen.

In France, by law, 2 weeks must pass from the moment of application to the plastic surgery itself. We don't have such a norm. In times of crisis, some surgeons were ready to operate here and now: “We’ll operate, and then we’ll paste in the tests.” A difficult question: either serve all patients, play on their feelings, or remain respected among professional colleagues.
