Leonid Leonidovich Pavlyuchenko

In 1967 he graduated from the Faculty of Medicine of the Lvov State Medical Institute, specializing in orthopedist-traumatologist.

From 1967-1975 - surgeon, traumatologist at practical healthcare institutions.

From 1975-1984 - junior researcher at the Research Institute named after. N.V. Sklifosovsky under the guidance of Professor V.P. Okhotsky.

In 1984 he defended his thesis for Candidate of Medical Sciences on plastic surgery of hand tendons. Having practical experience in transplanting tissues, nerves, blood vessels, tendons and scientific work in plastic surgery, he entered aesthetic surgery.

In 1984, he was invited to one of the three aesthetic surgery centers existing throughout the USSR - the Beauty Institute on Novy Arbat.

In the early 1980s, a special plastic surgery department with 50 beds was established at the Institute.

Until 1991, Pavlyuchenko was the head of the plastic surgery hospital at the Beauty Institute under the leadership of E. M. Dolzhikova.

Since 1991 - Associate Professor of the Department of Surgery at the Peoples' Friendship University of Russia under the guidance of Professor V. S. Dmitrieva. He was invited as a lecturer at the Department of Maxillofacial Surgery.

In 2000, Pavlyuchenko defended his doctoral thesis on the pathophysiology of liposuction and initiated the creation of a new department of plastic surgery and cosmetology. From the same year to the present (2015), Pavlyuchenko has been a professor at the Department of Aesthetic Plastic Surgery at the Faculty of Medical and Mass Media of the People's Friendship University of Russia.

Scientific interests of L. L. Pavlyuchenko are plastic surgery and pathophysiology. Until the 1980s, he researched the problems of treating injuries and diseases of the hand. Since the 1980s, scientific thought has been aimed at considering the issues of surgery for clefts of the upper lip and nose, plastic surgery of the ears, face, nose, plastic surgery of the mammary glands, and liposuction. L. L. Pavlyuchenko acts as a publicist, being vice-president of the Society of Plastic and Reconstructive Surgeons of Russia and a member of the editorial board of the journal “Plastic, Reconstructive and Aesthetic Surgery”. As president of the Association of Plastic Surgery and Cosmetology Clinics, he developed and introduced for the first time in Russia standards and methods of plastic surgery. Corr.: Leonid Leonidovich, in what directions is modern facial plastic surgery developing?

Leonid Pavlyuchenko: Today, two main directions have clearly emerged. The first is minimally invasive surgery. This type of operation includes various liftings using fillers or threads. This also includes operations on certain areas of the face. In such cases, only local anesthesia is usually used. The recovery period lasts 2-3 days. However, the effect of this type of procedure is short-lived by medical standards - only six months or a year.

As for the second direction, it is, on the contrary, a complication of operations that allow lifting of both skin and muscles, which lose their former strength with age. Of course, this is not a minimally invasive method, however, the effect of operations of this type will please the patient for 8 or even 15 years. We must remember that when choosing a particular procedure, you should take your age into account. The older you are, the more likely you are to require deeper surgery.

Corr.: As far as I understand, you specialize in complex methods that give a lasting effect. What types of facial surgeries do you highlight within your specialty?

Leonid Pavlyuchenko: I can distinguish three types of plastic surgery on the face. The first type is rejuvenation, the second is improvement of appearance, the third type is total transformation. By improving appearance I mean changing facial features, giving them greater expressiveness.

Corr.: What is the most important thing during an operation?

Leonid Pavlyuchenko: I can say with confidence that any of the three operations requires from the surgeon, of course, professionalism and perfect knowledge of facial anatomy. While transforming a person, a physician must under no circumstances harm the patient’s health. The medical team is also an important factor. It must include only qualified people, including, of course, an anesthesiologist. You can’t do without an operating room and a hospital. Everything should be equipped with the best technology. This is the golden rule.

Corr.: From those who are going to undergo facial plastic surgery, you can often hear that they are afraid of the “mask effect” that may occur after the operation. Are there any methods that will completely help eliminate such risks?

Leonid Pavlyuchenko: Of course there is. The “mask effect” occurred earlier when surgeons were only allowed to touch the sides of the face. Now there are no forbidden areas on the face for surgeons. The lift is performed along the entire perimeter, the tissue moves from bottom to top. The fact is that over the years the skin sag. It becomes smaller in the upper part of the face, everything is concentrated below: a double chin or sagging cheeks, the so-called jowls, appear. To restore youth and harmony to the face, we use several techniques. The innovation here is that we have developed a deep access technique in the temporal region. This helps protect the facial nerves from damage. You probably know that touching them leads to sad consequences, to various kinds of paralysis. I note that it has become a tradition for my clinic to use the patient’s own tissue to create lost volume in the right places. I am not a supporter of fillers and implants. This makes the face heavier.

Corr.: You raised a very important issue about the facial nerves. The successful outcome of your lifting operations indicates that you have developed a scheme for yourself that allows you to avoid damage. What exactly is your method?

Leonid Pavlyuchenko: It's simple. The main thing is not to perform any manipulations where the nerves actually are. To understand what I'm talking about, let's first consider what layers any face consists of. The very first thing is the skin and the fat layer that is located under the skin. Next comes the superficial muscle layer. It is under this that all the nerves lie. After that comes the layer of muscle itself. The final element is the subperiosteal layer - the deepest of all. I work with this last layer when the soft tissue of the face needs to be put back into place. In order to remove excess fat and create new fat, I intervene in the superficial muscle layer. Thus, I do not touch at all the layer that contains the facial nerves. Here's my secret. Despite the fact that many manipulations are carried out on the deep layer, the recovery period lasts about three weeks.

Corr.: Many women, upon reaching a certain age, are most concerned about sagging skin in the neck area. How do you usually deal with these age-related changes?

Leonid Pavlyuchenko: Here, of course, a separate operation is necessary to achieve the maximum aesthetic effect. In such cases, the task is absolutely clear - it is necessary to restore the angle between the neck and chin. For this we have developed a special technique. I call it the hammock technique. During the operation, we use threads to “stitch” the muscles under the chin, which creates a kind of frame - a “hammock”.

Corr.: Is there any age limit for plastic surgery?

Leonid Pavlyuchenko: There are no age restrictions. There are restrictions due to health reasons. During the operation, the person will be under anesthesia for 4-6 hours. This is a certain burden on the body. That is why the patient should not have serious illnesses. If we talk about age, the bulk of our clients are, of course, women 50-55 years old. However, there are various extreme options. I remember my youngest patient was 21 years old. I also operated on a lady who was 81 years old.
