Plastic surgeon Andrei Iskornev was in Yekaterinburg for one evening, who today tailors the faces of the Russian political elite and show business stars to his taste. We briefly asked an international class expert and member of the international confederation IPRAS, who owns the Platinental clinic with branches in Moscow and Kazan, from his companion, TV star Tatyana Arno. That is, these are people who worked here, in some large corporations, and then moved to the public service, and one way or another we are friends.

- How do they find you?

— The world of plastic surgery is very intimate. And, as a rule, information about some specialists who are allowed to visit a person is passed on by word of mouth. What is called “word of mouth” in our world and, in my opinion, is a more revealing story than advertising in the media. If you open any glossy magazine, there is always a beauty section, there are some ratings of surgeons, often they are purchased. The topic of plastic surgery has long become a show business, and I am very pleased that I and my team, which I lead in Moscow and Kazan, are trying not to fall into this.

In addition, we take on some free charitable work from TV channels. And there is nothing wrong with informing through the media that there are clinics that are ready, within some limits, to provide free services in the field of reconstructive plastic surgery to those people who really need it. When it’s not just some whim of breast augmentation or nose remodeling, but a necessary reconstruction.

— Do you often encounter reconstruction that is really needed?

— In the minds of most people, plastic surgery has become a kind of easy genre, but in fact, aesthetic surgery is just one of the subdivisions of reconstructive plastic surgery. We helped patients after explosions in the metro, and the last patient we had (Andrei Malakhov from Channel One brought her to us) was injured during birth and at 30 years old looked 80. And as a result, my colleagues and I performed a very complex reconstructive operation that allowed her to become more social. Because this is not a whim, but a means to return people to society. Of course, we earn most of our money from aesthetic plastic surgery, and this allows us to engage in some kind of charity.

— In my opinion, aesthetic surgery is mostly breasts, lips and butts. Is there anything else? Several years ago, the most unusual thing to me seemed to be the removal of ribs from the singer Cher...

— Everyone is interested in texture: Cher’s ribs, the rejuvenation of the elbows of some actress or the anal bleaching of a famous musician, etc. There are a lot of such stories, but people are not stupid either. And now even provincial girls who come to Moscow with the desire to find a rich husband understand that boobs don’t solve the problem. For example, operations such as the removal of Bisha's lumps are now gaining great popularity. This is when this lower third of the face narrows a little...

- So they are not already removing teeth?

- No, no one removes teeth anymore. If you look at Western Europeans, their faces are all a little triangular. We are people closer to Asia, typically with round and oval face shapes. But now I have noticed a lot of 30-35 year old men and women who fly a lot, communicate with the West and are also trying to bring themselves a little in line with some European standard.

The main and very interesting trend now is harmonizing surgery. There are many peculiarities of how to operate on a politician, how to operate on a show business star, how to operate on a young patient so that he or she looks more harmonious and Europeanized. And everyone has accents. If you look at Hillary Clinton, she certainly could look even younger given the opportunities she has. But she looks fit, neat and about 50, although she is a little older. This is intentional. Because if she looks like the same Glen Close, conventionally, then everyone will pay attention to her plastic surgery and be distracted from her political program.

— Is there a different approach to show business stars?

If we are talking about an aging superstar who is used to being very beautiful, making money with her looks (her face is her job) and we understand that it is a tool, then yes, we work a little differently here. Then this is a story about how to change time, how to look 35-40 at 60. And this is also normal, because now the speed of life is completely different. And a huge number of people by the age of 50 only achieve some success and financial harmony and realize that inside they are 30, but on the outside they look completely different from how they feel. Therefore, such women, with the help of anti-aging surgery, regain their youth, which they spent, for example, on something else.

— At what age, by the way, do these same representatives of show business, politicians and others start coming to you?

— Around 30. Girls used to get their breasts and noses done if nature didn’t endow them with them. If at 22-23 they go and pump so much that it is unrecognizable - this is not always a problem for patients. Often this is also a problem for those specialists who do this. They inject hyaluronic acid at hairdressers, or invite home-based cosmetologists, who just want to inject more and at a low price. We are fighting this. Both the Russian Society of Plastic Surgeons and the Department of Plastic Surgery of the Russian State Medical University, headed by Natalya Manturova, are also actively fighting this.

Now the implants have changed. And in the last five years, I've known very few girls who [pouts lips]. By the way, I now have a huge influx of ladies who are having their lips made smaller. Because it used to be fashionable to insert silicone fillers, which then look like you were sucked into a drain, and girls then surgically remove this to look like a lady, and not you know who.

- We have vivid examples in Yekaterinburg - socialites. The girls looked really beautiful at first when their lips were slightly pumped up, but they didn’t stop in time, and now they have two blotches on their faces.

“You know that it all starts with one spoon of sugar and ends with two doses a day.” You understand that it is very difficult to grow old as beauties. Here you just need to listen to the experts and trust them. So you came to me, everything is fine with you, and I will tell you that I will not do anything. I won't tell her to go to others, but she will go anyway.

- Complications from the most popular operations of preening girls - have they been reduced to nothing today?

— In fact, everyone has long proven that implants do not have any effect on oncology, but on the contrary: women who get them simply do the same mammography, ultrasound more often and detect cancer more often. Because people just take care of themselves. And the fact that implants do not cause cancer has been studied in America for probably 30 years. For your lips, you should pick up the sticker from the drug you were injected at the end of the procedure so you know what you were injected with, when, and how much. There is no need to pretend to be a fool: “Oh, I trust you, do what you think is beautiful!” You need to delve into the process. And take responsibility for your life - then you will not have any complications.

— To change yourself, there are things besides the surgeon’s scalpel - these are now safe drugs and technologies?

— Again, it depends on the hands of the person who does it all. For example, anesthesia has now become much safer. If previously many operations required patients to endure, now everything is done under light medicated sleep. It’s all non-toxic, safe, a person falls asleep and wakes up two hours later at home. Lasers have appeared that make it possible to do some work much faster and more sterile; some injectable drugs and fillers have appeared that have become non-toxic and dissolve in a safe way. That is, a lot of materials that make plastic more accessible and safe.

In the days of Soviet actresses, when the first beauty institute was opened and Lyubov Orlova had her first lift, it looked like this: the girl had a lift and everything was pulled in one direction. And now we are talking about a three-dimensional facelift, when all this is not pulled in one direction, but each zone is tightened in its own direction.

— I heard that mitochondrial rejuvenation is gaining momentum among VIPs.

- There is such a thing. This is an enzyme called telomirase - a drug that slightly affects the rate of cellular DNA renewal. And the main technologies are now aimed at such high-quality centralized rejuvenation. This is a drug that makes the cell age more slowly and renew itself faster. Because all we are made of are cells.

— Are stem cells still used in rejuvenation procedures?

- They are used perfectly. But for some reason they talk about stem cells only in the field of plastic surgery, when stem cells are actually one of the most powerful tools in cardiology, neurology and restorative medicine. The most powerful developments in the field of stem cells do not concern plastic surgery. We get, rather, only their fragments. And they are used in the treatment of leukemia, heart attacks, etc. When these cells allow the scar tissue of the heart muscle to be replaced with new cells. Why not? If you have a choice: either after a heart attack you are left with a scar on your heart, or the cells, like a patch, replace this tissue with new one, then this is very good.

The ethical component is also very important here. Many people confuse stem cells with embryonic cells, with cells taken from the human placenta. There's a lot of scientific confusion here, and in the minds of the average person, these are all the same stem cells, and it's kind of harmful because you don't know what's going to happen. In fact, it has long been known what will happen. They have more or less already learned how to operate them, but these are very expensive developments that are not yet available to everyone. But I am sure that this is the future. Now there are even printers that can print some fragments of internal organs from stem cells. This is all at the development level, but I would not be surprised if in 10-15 years we will have printers that print artificial skin from stem cells. Why not?

— Do you have an exclusively medical education? My colleague and I had an argument: should an aesthetic surgeon, who is partly a sculptor, have at least a semi-art education?

— I have a medical education and a business education in the field of medical management. At the same time, I draw well, yes. But I don't think that's the point. The techniques in surgery are essentially more or less the same, and they are all described. We attend all sorts of events... By the way, a brilliant congress is taking place in Yekaterinburg, which was first organized by the late Professor Nudelman and which was attended by both Russian and American surgeons. The difference between a good and average plastic surgeon is aesthetic sense. It's like taste: you can be a good dressmaker, or you can be a brilliant fashion designer.

— A separate block is a gender change. Do we do such operations?

- Undoubtedly. Gender reassignment surgery is a very serious and big story. First, you must understand that this is not some kind of whim. These are people diagnosed with transsexualism, nuclear form. And there are two options: either the person will jump out of the window at some point, or he will bring himself into line with the gender in which he recognizes himself. These are not homosexuals, not transvestites, but people who, from childhood, from the age of 5-6, feel themselves in another body. And before undergoing such an operation, they undergo a very tough and lengthy detailed psychiatric commission, which actually conducts an examination that this is not some kind of psychiatric disorder, but really a diagnosis.

I completed my residency in the department of Academician Milanov - this is our greatest plastic surgeon, who was engaged in successive gender reassignment operations. And I want to say that in Russia there are crazy developments in this regard. We carry out gender reassignment operations that have no analogues at the world level. But they're just not sponsored that way. All this, of course, comes down to money. Previously there was a quota, but now not so much, as far as I know.

But the point is that when a patient comes and already has a legal basis to perform such an operation, the surgeon agrees to perform it on the principle of minimum sufficiency. That is, if a boy wants to become a girl, then not all stages are done at once, but only some - in the hope that at some stage he will stop and this will be psychologically acceptable for him. That is, the operation does not always begin immediately with the removal of the penis or uterus. Because (again) some operations are very traumatic, and some not so much. Therefore, this is a very big international problem, a diagnosis that plastic surgeons deal with.

— Are there many transvestites in Russia?

- Yes, there are a lot of such patients, you just don’t notice them. Transsexuals are not some American Amanda Lepur who undresses in public and shows her tits on stage. And this, for example, will be a lady standing at a bus stop in a gray sheepskin coat, inconspicuous, without any special features. Usually such people change cities, get married, and the husband does not always know. They then undergo treatment for a long time, as if for infertility, and then they simply adopt a child and live as a harmonious family. And no one ever knows about it.

By the way, there are statistically more operations when a woman turns into a man in our country. Everything is clear there: by the neophallus, that is, by the newly modeled penis, of course, it is usually noticeable. But there it is different, there the psychological satisfaction of such a man is that he can satisfy his female partner as a man. That is, they usually live on testosterone.

- Sorry for the intimate question - is it worth it?

— There are prosthetics that allow it. This, by the way, is also used in men who suffer from impotence. But this problem exists more in the brain. There is a special pump in the scrotum. And there are also operations that make it possible to transfer the sciatic nerve in this way to the neophallus in order to experience sensory sensations. Not everyone succeeds, but it happens. And this is already a complex reconstruction.

— What is the price of becoming a woman from a man and vice versa? Approximately. Are these millions of rubles?

— Somewhere around 1 million, probably. It all depends on the implant. You must understand that the singer who participated in Eurovision underwent surgery for a much larger amount to look the way she does now. And if this is some kind of minimal adaptation and a woman simply removes her breasts, her uterus, reconstructs her neophallus, does some kind of muscularizing surgery on her face, then it doesn’t cost that much. It is clear that this cannot cost 50-60 thousand - this is a fact, because there is a consumable material that, fortunately, is not produced in Russia. We buy this in America, and although it is now unpatriotic, factories there have been producing breast implants for 30 years.

— I was told here about our clinics that purchase anti-cancer injections from Russia and only increase mortality, while drugs purchased abroad give hope for remission.

— Yes, this is a big controversial issue that needs to be raised. And when they say that they will not purchase any medicines now, this will, of course, be a great tragedy for our country. I also believe that generics and antibiotics... All patients buy thyroxine in Europe. Those who fly are middle class. And I don’t understand why there is such a problem with L-thyroxine, which in general is not difficult to synthesize. But the effect they get from a pill bought in Paris is different from ours.

— Is this a disaster for you?

- Yes. Because the anesthesiologists and I have virtually no components other than narcotic drugs that are used in Russian-made anesthesia. So that the patient falls asleep, and wakes up an hour later and writes SMS - this is all done using foreign medicines. The bandages that I use during the facelift process - again. Now it’s unfashionable to talk about this, but I say: the Russian napkin is falling apart in my hands and it’s all tattered, all in pieces, and can no longer be used. Or I take an expensive German one, and I can use it for half an operation, and it doesn’t fall apart, it’s stitched. And I don’t understand why you can’t make a quality product, why make some kind of crap? “And it makes me very angry.” That's why I actually work in private practice - intentionally. Because I want to be able to use the best medicines, tools, and I want to attend conventions all over the world. And I don’t want to save on my knowledge, which I then sell to my patients. But I pay money for it, and it’s no secret. And if you have already gone down this path, then you must be the best at it, and not just plug holes.

— Our elite, whatever one may say, has not become catastrophically impoverished.

— I don’t like this story about the elite at all. Because these people's operations are no different. You can’t imagine how much I hate the stars when they come to me with their show-offs. A serious star never shows off, because these are hard workers who live in very harsh conditions. And if this is some kind of upstart, then the exit is in the same place as the entrance. I have a totalitarian regime here in the clinic. I can't stand it. Because you trust me and should take the pill exactly after 12 hours, and not after 13. Because, God forbid, you will then go to Channel One to tell Malakhov that you were treated incorrectly. I don't have any democracy here.

— Remember which administration people come to you from more often—the regional or the city?

- I will not say. They pay me money for this so that I don’t say such things. True, many people come from Yekaterinburg. I understand that you are very interested in all this, but believe me, patients from Yekaterinburg are no different from others. Everyone has the same goals: people want to look well-groomed and fit. And this is a completely standard package - facelift, endoscopic forehead lift, rhinoplasty, injections.

“Everything has long been cut out, removed and tightened up.” I use everything! And I’m not ashamed to talk about it.

— I wanted to understand what you would change in every second?

— I would change the weight. Stop eating, my dears! Because the cheapest way to look young is to have a good and correct weight. Being thin is actually useful in terms of atherosclerosis, hypertension, which suddenly appears in all 30-year-olds. And I would first of all deal with this, and only then, as I stop liking something, I need to solve it using other methods.

— Now the most popular disease is cancer, which somehow unexpectedly began to grow.

— There are many factors for this. Look: every second person in our country does not remove a cigarette from their mouth. Smoking is banned all over the world, but here we are like fools. And you can’t prohibit it here: you just have to admit to yourself that if you smoke, then you are a smoker. If you drink half a bottle of wine every day, then you are an alcoholic. You can tell stories about how great the Italians or the French drink, but they drink a maximum of 100 ml a day and with good food, and everything else is alcoholism. And we don’t promote sports among young people at all. By the way, I recently talked with Irina Rodnina, and they will also be doing this now, and I support them. Maybe, of course, the story of the GTO sounds a little Soviet, but the fact that sports need to be returned to the masses and educational culture is very important. Here's cancer for you: what we eat, what we drink, how we spend our free time. You choose your sexual partners, so why don't you choose what goes into your mouth from a plate of food?

— We recently chose the next annual beauty standard - Miss Yekaterinburg. What is your benchmark? Of course, after Tatiana Arno, with whom you were spotted here in Yekaterinburg.

— I like Katya Gracheva, presenter from Russia-24. Very beautiful girl. For many young girls in Moscow, I know, the standard is the head of our state.

— By the way, about the top officials of the state. You can’t convince me that they don’t do some anti-aging things for themselves. I want to understand: from your point of view, they do it all skillfully, beautifully? And where - in the clinics of one patient?

“Some of the public figures are doing this outside the country so that there is simply no leakage of information, but I am sure that everyone we see in the frame is doing something to themselves. This is definitely true. It’s just that some people do it in an easy way, others in a “serious” way. And that's okay.

— Well, our politicians traditionally go to Israel or Germany to treat cancer...

- You know, I would also go to Israel for cancer treatment, although it sounds terrible, of course. I just think that some areas in our country still need additional investments and specialists. There are brilliant Russian oncologists, and there are excellent centers that do this, but sometimes in a stressful situation it is simply impossible to find them. And statistically, my patients who have the opportunity to leave fly there to treat these problems.

— I believe that Leonid Mikhailovich Pechatnikov was originally a general practitioner.

A young, brilliant surgeon Volodya Nosov, an oncogynecologist, works in the oncogynecology department of a private medical center in Moscow. And I think he is already famous and will be a superstar in the future. Very talented. So yes, they are cool. I have a whole book where I write down simply the best of the best - there are already about 50 people. And with some of us we really can’t stand each other, but I still give them their contact information because I know that there is no one better.

— How to choose a clinic and surgeon?

— I think that you need to listen to your intuition. There is no competition between plastic surgeons at all, because each patient will choose his own doctor. And if you choose a plastic surgeon, then don’t be too lazy to visit several. You will 100% coincide with someone in terms of beauty, but you won’t like someone very much. And listen to yourself - this is normal. And maybe even one and the other will perform the operation normally, purely technically. But with one you will recover quickly, and with the other slowly. Therefore, it seems to me that psychological contact is very important here.

— Some of my colleagues have in mind that now they will turn 35 (and some are already 40-45) and they will simply be forced to rush to plastic surgeons.

Plastic surgery is about self-confidence. You need to use it exactly until you reach 100% self-confidence. This is the story of Barbra Streisand's nose, which is already the talk of the town. Enough time has passed, and a girl with whom he is already connected by a common past has returned to Andrei Iskornev’s life. “We have both changed a lot. I think that these are positive changes that influenced this come back...” admits Iskornev.

Andrey, your heart is not free again... Share who she is? The girl's name is Irina again, although I call her Irena. It so happened that this was a reunion after 8 years of separation and an even longer friendship. Ira and I lived together when we were both studying at universities. Then we started working together, Ira was my assistant then. Then we broke up, she went to work as a producer for a large publishing company, and returned a few years later. We started talking and working again. This time, Ira, with her experience, headed the marketing and IT departments at my Platinental clinic. And it turned out that the work romance again turned into life.

Tell me how you got back together? Ira and I have always been very similar. This was clear from the first days of our acquaintance. And a sense of humor, and perception of the world, beauty... and everything! We are sociable, although sometimes we are a little misanthropic, we notice certain things in the same way, we say the same thing at the same time. This is exactly the case when everything became clear from the very beginning.

Why did you break up 8 years ago? We lived together for a year. Life was quite difficult; we rented a small one-room apartment in a very strange house. We nicknamed it the “round house” because of its donut shape. (This is a project of the 80s, when in Moscow they wanted to create several such houses, the number of Olympic rings, but only one was built.) It was not even a one-room apartment, but some kind of “half-room” apartment. Our kitchen could only fit two people and an ironing board. And it was an amazingly fun time! We were always joking around, still managing to invite guests. And once we almost got a French bulldog, which the owner refused. Then she came to her senses and took the dog, but during these two days the dog smashed the apartment to smithereens. (Now my Labrador Barbie occupies a special room the size of our apartment.) Then it turned out that we broke up. I won’t go into details, it was difficult and difficult. But gradually we reestablished contact. Apparently, we needed this time to gain life experience...

They didn't talk about the wedding

Were there any doubts about whether to rekindle the relationship? Did not have. Everything turned out so logical and natural! Ira courageously supported me after the end of my television novels. When I returned to my normal life and schedule, we no longer had time for these memories. They wrote about us on the Internet before the start of the project in Ukraine, but everything there was not true: we have no children, and we have never been married.

Do you think this is your true love? I am so careful with this word. Probably for the first time I can say with confidence that it is she.

Are you living together now? Let's live. Ira spends a lot of time with me outside the city, and I increasingly take her on my professional trips. But at the same time, we each enjoy spending a couple of days a week on our own territory. I think we need to take a little break from each other.

Is your girlfriend jealous of your fans or Irina Skorikova? Irene is one of those who absolutely do not care what happens to me within the framework of some kind of social life. We have an understanding of a fortified house, and everything external is often just a reason for jokes. Of course, she is not jealous of Ira Skorikova. By the way, she now has a well-established personal life in Germany...
