Lipofilling technique
The operation “lipofilling” appeared in the mid-80s. One of the first surgeons to describe the technique of injecting a patient’s own fat into areas with missing volume was the Frenchman Pierre Fournier. Initially, the method was used to correct “blockages” after previously performed liposuction: semi-liquid fat was injected through cannulas and filled the voids left by aggressive fat harvesting. Next, surgeons improved the technique and began to inject autologous fat into the area of the face, chest and buttocks. Over the course of many years of work with fat, not only the approaches to the operation have changed and improved, but the process of fat collection itself has also changed.
More than 35 years have passed since our colleagues in the workshop began to use the patients’ own biocompatible material - subcutaneous fat, fat collected during liposuction. Today we work not with classical instruments (cannulas), but with an entire device created specifically for the needs of surgeons and patients - the BodyJet water-jet liposuction device. Working with this instrument made it possible to increase the percentage of fat retention: if previously, several months after the operation, about 30-50% took root, today with the BodyJet device we get better results - up to 70%.

Breast lipofilling surgery is performed in two stages:
- Removal of fat tissue The markings on the day of surgery indicate the areas of liposuction - areas for fat collection. Most often, excess subcutaneous fat is observed on the abdomen, sides, hips, knees and back. Using water-jet liposuction, the surgeon performs gentle tissue detachment, which ensures a more comfortable recovery for patients after surgery (compared to earlier fat harvesting methods). The collected fat remains in a special reservoir - a lipocollector for further transplantation into the area requiring correction.
- Injecting fat into the mammary gland area
In the second step, the surgeon begins to inject the collected fat into the mammary gland area. During the operation, hypercorrection of the breast is intentionally performed (a larger volume of fat is introduced), taking into account the lower survival rate of fatty material (about 50-70%).
Abdominal liposuction
We use liposuction and lipofilling for body contouring.
The essence of the method is as follows: under anesthesia, a special solution with painkillers and vasoconstrictors is injected into the “problem areas” of the subcutaneous tissue (where there is excess adipose tissue) to the patient. When the solution is evenly distributed and spasm of the small vessels of the subcutaneous tissue occurs, the solution along with the adipose tissue of the subcutaneous tissue is sucked out with special cannulas (thin tubes) through punctures in the skin.
Fat cells in an adult do not normally divide. A person's obesity is determined by the fullness of fat cells, and not by their large number. Therefore, liposuction is a reliable method for removing fat from areas where diet and exercise cannot get rid of it. New fat cells will not grow.
The liposuction method allows you to evenly remove excess fat deposits. Traditionally, this is done on the stomach, hips, buttocks, and lumbar region.
How is the operation performed?
Lipofilling of the mammary glands is performed under anesthesia (general anesthesia) and lasts about 3 hours. The duration of the operation is determined by the surgeon’s two-stage work: primary fat collection through gentle water-jet liposuction using a BodyJet device and injection of the resulting autologous fat into the mammary gland area.
The operation of breast augmentation with the patient’s own fat gives high aesthetic results. Very often, lipofilling is combined with other plastic surgeries: in some cases, the technique is combined with breast augmentation with silicone implants. This is especially true for those patients who have a small volume of their own breast tissue. The introduction of autologous fat allows you to hide the visible contours of the installed implant and, as a result, get natural breasts with a beautiful natural transition along the border of breast prostheses. The method can also be used delayed: several years after classical endoprosthetics, if the patient has lost a lot of weight and the contour of the prosthesis is visible, or rather large implants were initially installed (to mask protruding contours).

Lipofilling of the mammary gland, breasts, buttocks
The fat obtained through liposuction can be used to “add” it to those places where it is “deficient”. This is the essence of the lipofilling method.
Fat taken from the patient’s abdomen or other “problem areas” can be used to enlarge and reconstruct her mammary glands, to give roundness to her buttocks, and to smooth out wrinkles on her face.
An important point of lipofilling is to ensure that the transplanted fat survives in a new location. If you transfer too much of it to one place at once, it will not take root and will die due to poor nutrition; if too little, one procedure will not be enough.
There is a whole series of measures to ensure the success of fat engraftment during lipofilling: this includes special equipment for liposuction, special instruments, separators for fat cells, medications, etc.
We use lipofilling to enlarge the mammary glands for cosmetic purposes (including for the correction of different breast sizes), for skin preparation and contouring when restoring the mammary gland (after its removal), to correct the shape of the buttocks.
Postoperative period: recovery
After breast lipofilling, patients remain in the clinic for a day. Immediately after the operation, special compression underwear is put on the liposuction area, which prevents the development of swelling and the appearance of extensive bruises and hematomas in the areas of fat sampling (underwear must be worn for 3-4 weeks).
Special bandages are glued to the chest area, which are changed at each dressing. The dressing schedule is discussed individually (usually dressings are prescribed at intervals of 2-3 days). Among the special recommendations: in the first week after surgery, patients are prescribed a special high-calorie diet to preserve a greater percentage of the fat injected into the breasts, and the operation area should not be overcooled!
The first month after breast lipofilling, you should avoid sports and active physical activity. Among the special restrictions: for up to 6 months, patients are not recommended to visit baths, saunas, solariums, and when exposed to the sun, especially during the beach period, they should actively use sunscreens with a high degree of SPF protection, especially in the area of surgery.
Liposuction for false gynecomastia and additional lobules of the mammary gland
In addition to these areas, we have experience in using liposuction to reduce the thickness of adipose tissue in the axillary areas of women - which is often mistaken for additional lobules of the mammary glands; on the chest in men - with false gynecomastia; in the back of the neck - with a cervical hump.
Liposuction is a cosmetic surgical procedure. Excess fat deposits are not a disease, and insurance companies will not pay for surgery to remove them. The cost of liposuction from one zone is 10-15 thousand rubles.

2 months after lipofilling, we evaluate how the transplanted fat has taken root: if it is not enough, we repeat the procedure of liposuction and lipofilling. If there is enough integumentary tissue, we plan an operation to install an expander.
Transplanting a lot of fat at once is risky: without receiving proper nutrition in a new place, it may not take root and die. Replanting a little at a time will obviously extend the reconstruction time. Special preparations that we recommend to our patients improve the survival of fat. Significantly worsens the survival rate of fat - smoking. It actually nullifies the effect of all medications prescribed by the doctor.
