Having told him about me, the site and the specifics of online communication, she achieved a response - the surgeon wanted to meet with me and, after personal communication, invite me to watch and photograph some operation. Anna and I agreed to go together: she for the examination and I for the meeting. (If you read all my stories about filming operations, you may get the impression that not a single plastic surgeon has ever seen Katya not sleepy. The examination was scheduled, naturally, for the morning, and the night before I did not manage to go to bed. In the car, as soon as I closed my eyes, I immediately fell asleep (don’t think that I was driving :)). Therefore, I entered the clinic in a state of “waiting mode,” and Anya and I sat in a cafe, trying to cheer up with coffee, while Blokhin talked with other patients. When he finally appeared at the end of the corridor, my brain gave the command “start” and I tried to feign energy and interest.

In life, Blokhin turned out to be absolutely the same as on the screen (before we met, I had already seen him in the first episode of the reality show “Formula of Beauty”): straightforward, sharp (“What’s wrong with your teeth?” he asked about my braces for the entire cafe, taciturn (but I could have a philosophical conversation on this topic) and a sense of humor not for vulnerable girls. The conversation was short, and the agreement was specific: in a week I will come to the same clinic (Blokhin works approximately four different ones) - photograph augmentation mammoplasty.

Two with access through the armpit and one through the areola. I drove with curiosity. After all, I have never seen such endoprosthetics as Blokhin promised me. And I, indeed, have never seen implants placed through the areola. I just heard the story of one of my friends, who has the most imaginative idea of ​​​​plastic surgery, how she saw “disgusting” on TV: the nipple is cut off, raised, an implant is pushed into this hole, and the nipple is sewn back into place. To be honest, this is also how I imagined this operation. But it turned out…

The first operation was supposed to be mammoplasty with installation through the areola, but I had to wait a little. Or rather, three hours until one of the operating rooms is free. The operating rooms in the clinic were impressive - very spacious (you can spin around the table as you like, climb on a stool, without getting close to the sterile table with instruments), with automatic doors and clearly the most modern equipment.

The patient (like the next two) is thin and has small breasts without ptosis (drooping). Blokhin explains that in her case the implant can be installed in combination, or under the gland, with access through both the areola and the armpit. Selects a combined installation with access through the areola. The patient had no preferences in this regard and completely relied on the surgeon (apparently she is not interested in the Internet).

An incision along the areola is made in a semicircle and then Blokhin begins to peel off the skin. It turns out that normal plastic surgeons never install an implant “through and through” through the areola, since this traumatizes the gland too much and can lead to damage to the milk ducts (which is what surgeons who do not know how to properly enlarge the breast in this way like to scare).

In fact, after the incision, the skin is detached down to the submammary fold (the place under the breast) and the gland rises there. That is, the implant is inserted in almost the same way as clearly when accessing under the breast, only without this obvious scar, since the distinctive feature of the areola access is that it heals almost without a trace, especially if it is covered with tattooing. The only inconvenience, according to Blokhin, is that from the nipple to the submammary fold, at the site of skin detachment, irregularities (small tubercles) may remain for some time, which resolve in about 4 months. Then everything is as usual. The gland is lifted, the muscle is “torn off” (which requires quite noticeable effort) and the implant is inserted. In this case, round Eurosilicon (Blokhin prefers to work with Eurosilicon and McGan). The operation takes place extremely quickly - in half an hour. I think it is unrealistic to install an implant through the areola faster. In ordinary sewing, painstaking work can be the key to quality, but not in plastic surgery. The faster (without compromising quality, of course) the surgeon works, the better the result of the operation and the easier the postoperative period.

Between operations that follow one after another, Blokhin has a consultation - the patient was two hours late, so she doesn’t have much time to talk. Blokhin takes me with him, introducing me as an assistant (a good assistant - she steps on her pants, which are tightened somewhere under her chest and the shoulders of her robe hang approximately in the middle of the shoulder). I modestly sit down on the couch by the window and look at the girl. Problems: ptosis, “empty” breasts, wide chest, thinness. Wants to increase to size 2. But Blokhin stops: “To fill out your breasts, you need to bring them to 2.5-3 sizes.”

The girl wants a combined installation: she read on the Internet that an implant with a similar installation descends more slowly. “This is nonsense,” Blokhin objects. – Descends at the same speed. There is no ideal setup - you have to choose what suits a particular person. You are shown an implant under the gland. If we put the upper edge under the muscle with your ptosis, then over time the breast will “drain” from the implant.” I’m tempted to tell you more about the “double bubble”, but it’s not polite for the “assistant” to interrupt the surgeon (with difficulty, but I remember it). Only anatomists recommend using McGan’s “matrix” in this situation. “Although anatomical implants can occasionally turn over (what causes this is not always clear, usually due to individual differences), in this case, the top of a round implant would be contoured too much.”

“What kind of access will there be?” - asks the husband present at the consultation. “In this case, you can only do it through the areola, but with your nipple color there will probably be little white scars that will be difficult to cover with a tattoo.” I have already noticed that Blokhin does not like to hide possible negative consequences from patients. “Can’t I see the photographs?” But Blokhin no longer has time - the second patient is on the table. “What will you see there. Anyway, I’ll show you only the best work, and you’ll never see identical cases to sculpt your breasts in the future.” Here I don’t entirely agree - sometimes the best people show such results that you immediately want to run out of the office. However, now the patient herself was to blame - she did not arrive at the appointed time.

The second patient is on the table - installation of round Euro-silicon implants through the armpit. During the operation, the radio is on, on which they constantly play advertisements for one of the well-known Moscow clinics: “I changed my appearance in just two hours - they pumped out the fat from my waist and placed it in my chest.” In principle, I can imagine how tempting this must sound for women who have little idea about plastic surgery: she has lost weight and everything in her breasts is natural. What happens after such manipulations, of course, will not be discussed in advertising. They may not even tell you at the surgeon’s appointment. Transferring a significant amount of fat causes inflammation, encapsulation of fat and its uncontrolled migration, as a result of which the breasts take on different (bizarre) shapes.

The second operation is also done in the shortest possible time, and we go out for a smoke - that’s about the time it takes to take the second patient out of the operating room and bring the third one. Along the way, mistaking me for a local nurse, one of the staff reprimands me for the lack of clean shoe covers in the tank... “So, maybe I can get you a job at the First Medical Center?” - Blokhin is interested. “Yeah, then by the end of it I’ll be able to pretend to be a student of the most famous plastic surgeons. True, with all the graduate studies, I will be about 40 years old.”

There are some difficulties with the third operation: due to the individual characteristics of the patient, she can only have an anatomical implant installed and only through the armpit. The difficulty lies in the fact that it is extremely difficult for the anatomist to place it through this access - it is almost impossible to understand whether he is in the correct position, being inside the chest. To determine this very position, all conventional anatomical implants have such a notch on the bottom. By feeling it, the surgeon understands where the middle of the implant is. But when installed through the armpit, you cannot touch the bottom of the implant. There is one way out for this situation - the Kamora implant from Eurosilicon. Its feature, designed specifically for this situation, is that the “notch” is made on top. This allows you to avoid an ugly (according to Blokhin) scar under the breast (the girl’s areola is too small).

The last operation was completed successfully. All together, including breaks between operations, it took only three hours. And I’m going home so physically exhausted, as if I had the operation myself. It’s scary to imagine how much endurance surgeons who constantly operate must have.
