The results obtained give reason to believe that the treatment of pathological skin scars using electrophoresis with Fermenkol has therapeutic effectiveness . Electrophoresis of the collagenase preparation promotes a statistically significant regression of such parameters as the type of scar (by reducing the severity of hypertrophy); consistency of the scar (its density decreases); color of the scar (hyperemia decreases, the erythematous color of the scar becomes less intense). It also tends to reduce the severity of subjective sensations : tension, soreness, itching. Analysis of photographs of patients before and after a course of therapy shows a visual slight decrease in the severity of scar erythema, as well as a tendency to change its configuration due to the involution of its least hypertrophic areas. At the same time, the effectiveness of scar treatment with electrophoresis with Fermenkol depends on the age of the scar: thus, the consistency of a “young” fragile scar becomes significantly less dense, while stronger old scars practically do not change in consistency. However, even scars more than 12 months old are also susceptible, although to a lesser extent, to the action of the drug: thus, patients note a decrease in the tension of the scar, and in the presence of contractures, an increase in mobility, the degree of extension in the joint covered by the contracture.

The clinical sign of a scar, the most sensitive to the effects of therapy, turned out to be subjective sensations. Thus, the vast majority of patients primarily indicate a decrease in tension, pain and itching of the scar (by 50%; Fig. 1). The next most important sign is the severity of erythema (by 36%; Fig. 1). A change in this feature can be seen when studying photographic materials. Also, when palpating scars, you can notice a decrease in their density under the influence of therapy (by 25%; Fig. 1). Finally, the change in scar type (by 22%; Fig. 1) is significant mainly for young scars (Fig. 4), and always occurs more slowly than changes in other parameters.

According to the results of a study of the comparative effectiveness of therapy for pathological scars in the main and control groups of patients, it turned out that the maximum effectiveness of therapy for pathological skin scars occurred when using electrophoresis with Fermenkol . Only this drug reliably changed all the clinical parameters we studied.

The next most effective treatment was a course of phonophoresis therapy with the drug Contractubex. The consistency of scars decreased slightly (by 24%, Fig. 2), erythema decreased (by 31%, Fig. 2), subjective sensations were comparable to those during fermenkol therapy. These changes turned out to be statistically unreliable. Manufacturers recommend using this drug in long courses, coinciding with the timing of the physiological involution of the scar, by simply rubbing it into the scar. The use of phonophoresis to administer the drug could ensure its deeper penetration and, accordingly, greater effectiveness in shorter periods of time. Also, the very effect of ultrasound on scar tissue cannot be ruled out; in modern physiotherapy, ultrasound therapy is widely used to treat scars.

The use of electrophoresis with lidase affected only the subjective sensations of patients, practically without changing other parameters (Fig. 3). Thus, the widely used therapy of pathological scars with hyaluronidase preparations was ineffective in the present study.

The main task of treating pathological skin scars is mainly to change the type of scar, to transform a hypertrophic, keloid scar into a normotrophic one. However, the results of the work show that this particular parameter turns out to be the most rigid and less susceptible to regression. Thus, the regression of scars after the use of one course of Fermenkol electrophoresis, which includes 10-15 procedures, does not have sufficient visual severity, as indicated by the analysis of photographs of patients. This necessitates the use of not one, but several, even many repeated courses of enzyme therapy for scars, in combination with other pathogenetically based methods of therapy. Also, from our point of view, an indirect indicator of the effectiveness of Fermenkol can be the fact that patients preferred Fermenkol therapy , as evidenced by the maximum number of patients in the first group.

Due to the fact that these enzymes have proteolytic activity, they are active against each other, as a result of which the concentration of enzymes in the solution decreases over time. The introduction of 2 mM calcium chloride as a stabilizer allows the enzyme activity to be maintained in solution for a sufficiently long time, which is especially important in cases where the entire volume is not consumed immediately, but gradually.

Giving the solution a pH of 5.0-5.5 increases the efficiency of administering the drug by electrophoresis. We used a solution of the drug with a concentration of 0.1 mg/ml, as the most economical and recommended by the manufacturer as optimal for typical applications. The required volume of solution was calculated as follows: per 1 cm² of scar, approximately 0.5 ml of Fermenkol solution is required. The pad was moistened with Fermenkol solution, the enzyme was injected from the anode, the current density was no more than 0.1 mA/cm² when the scar was localized on the torso or limbs and no more than 0.05 mA/cm² when the scar was localized on the face. The duration of the procedure is about 20 minutes (no more than 30). The duration of the course for correction of hypertrophic scars is 10–15 procedures.
