Description of Monobloc®CMC bioimplants
The history of the creation of a new generation implant filled with a biocompatible gel began in 1984, when the ARION company came to the conclusion that it was necessary to study an alternative filler to silicone that had cosmetic characteristics similar to silicone. The implant filler had to be biodegradable and thus easily excreted from the body.
The filler chosen was carboxymethylcellulose (CMC), a natural polymer derived from cellulose (a polysaccharide). CMC has a number of properties that make it an ideal filler for implants. It is soluble in water or saline and forms a clear gel that remains stable with changes in temperature, and the viscosity of the gel is maintained over time. The viscoelasticity of the gel gives the breast implant a similar appearance to natural breasts.
Carboxymethylcellulose is a safe and completely non-toxic substance for humans. First of all, it is a natural product of photosynthesis that occurs in all plants. Moreover, cellulose makes up about 80% of the pollutants we inhale throughout our lives. Secondly, cellulose is the basis of cotton products, such as clothing and medical dressings (bandages, napkins), and both of them have contact with human tissue without causing irritation or other negative reaction (this is especially important when in contact with a damaged surface) . CMC has been used in the food industry for more than 50 years (thickener, food additive E466), and also as the basis for many medicines and materials used in medicine (included in the US Pharmacopoeia (USP) and Europe (EP) and the State Register of Medicines of Russia under number 68 /333/16), for example: eye drops, plasma expanders, anti-adhesive agents (Seprafilm, Oxiplex). Due to its unique properties, carboxymethylcellulose gel is used as a medicine for adhesive disease: thus, on the basis of CMC, a new domestic anti-adhesive gel Mesogel was created, which is approved for use in the Russian Federation and is successfully used in operations in surgery and gynecology.
Dow-Corning in 1992-1994 compared the toxicity of silicone gel with carboxymethylcellulose and came to the conclusion that the latter is an absolutely harmless non-toxic material compared to silicone. So naturally, Arion's carboxymethyl cellulose may be one of the safest implant materials available.
When the gel enters the body, in addition to the absence of negative effects, it is absorbed. Unlike non-absorbable or poorly absorbable hydrogels, CMC easily decomposes into simple components such as glucose, carbon dioxide and water.

It remains to add that the choice of implant depends on the specific clinical situation, the condition of the bone tissue, the individual characteristics of the patient and other factors. Therefore, only a specialist in the clinic can choose it correctly.
Implant shape: teardrop-shaped and spherical
When choosing the type of surface, everything is more or less clear - a smooth surface is worse than a textured one, since there is a higher risk of developing long-term aesthetic complications. There is also certainty with the filler - cohesive silicone or Soft Touch is much better than saline solutions. They are also superior to carboxymethylcellulose, which is the basis of hydrogel implants.
Choosing the shape of an endoprosthesis is a very responsible and complex task. Breast implants can be spherical (round) or teardrop-shaped (anatomical). In the first case, the prosthesis has the shape of a hemisphere, the highest point is located in the center, the filling of the upper and lower poles is almost the same. In the second case, the point of maximum projection (height) is located in the lower hemisphere, which is more voluminous than the upper, which is why the implant takes on a teardrop shape.
Anatomical implants more accurately replicate the natural shape of the mammary glands, and this is their advantage. However, the plastic surgeon does not always recommend choosing a teardrop-shaped prosthesis. Firstly, it is much more difficult to install than a spherical one. Secondly, even a slight rotation of the implant leads to breast deformation, requiring repeated correction.
Drop-shaped implants are preferable to use in the following cases: breast asymmetry, the presence of post-lactation or age-related breast sagging, the need to recreate the natural shape of the bust during reconstructive surgery (after radical removal of the mammary glands). Also, anatomical breast implants can be recommended for women with a naturally wide interbreast space.
Spherical endoprostheses are preferable when it is necessary to achieve a significant increase in size, visually raise the upper segment of the breast or increase the fullness of the décolleté area. The advantage of such prostheses is that when they are rotated, the aesthetics of the breasts do not suffer - the shape of the mammary glands remains ideal. In addition, round implants are cheaper, and this also has to be taken into account.
Regarding the naturalness of the result, it is difficult to give preference to one or another option. It is believed that after the installation of round implants, the breasts do not look so natural, but there are some nuances here. Breasts enlarged with teardrop-shaped implants look less natural in the lying position, since they retain the same shape as in the standing position. In addition, spherical implants are also not completely round, due to which a natural increase in the filling of the lower hemisphere is observed in the standing position.
Ultimately, when choosing between round and teardrop-shaped implants, the following individual patient characteristics play a decisive role:
- woman’s height, body features;
- the shape of the chest, its height and width;
- initial breast shape and size;
- the presence of asymmetry, signs of post-lactation or involutional ptosis;
