Prolapse of the genital organs in women: prevention, symptoms, diagnosis and treatment

The pubic bone is the main element of the pelvic girdle, located below in front. The structure of this element is different in men and women, but the stronger sex is less likely to experience discomfort due to the large size or protrusion of the pubis. When the pubic bone protrudes strongly in women, painful sensations appear in the very lower abdomen above the urethra.

An abnormality with protruding bone should be distinguished from pathologies that cause pain. A condition without pain in most cases does not cause harm to health. Discomfort can be a sign of illness and serious injury.

What is female genital prolapse?

All pelvic organs (bladder, uterus and rectum) are supported by a peculiar arch of muscles, ligaments and fibers, which are fixed by the bony anatomy of the pelvis. When the arch weakens, the structures at the top fall down. The condition cannot be triggered because it does not go away on its own. The smartest step is to contact a specialist who will diagnose prolapse of the female genital organs and prescribe adequate treatment.

There are 2 types of prolapse : asymptomatic and symptomatic . The first means that there is no exit of internal structures into the vagina. Symptomatic prolapse refers to those cases where organs pass through the vaginal opening to the outside. Although the condition does not pose a serious threat to life, it can worsen the quality of life, significantly affecting a woman’s feelings and self-esteem.

Pelvic organ prolapse can be very uncomfortable, but there are many treatment options that can improve symptoms. In some cases, pathological changes can be eliminated completely.

Among the treatment methods :

  • use of pessaries;
  • physiotherapeutic procedures;
  • hormonal pharmaceuticals;
  • pelvic floor training;
  • surgical practices.

In the United States, 200,000 operations are performed annually to correct pathology. In Russia, such statistics are not publicly available, but it is known that the number of women suffering from the anomaly is growing. If before the age of 30 the disease manifests itself in every 10 women, then after reaching the 50th birthday every second representative of the fairer sex suffers from it. The high frequency of relapses after treatment allows us to consider prolapse of the internal genital organs as a chronic disease.

Indications

A man can choose liposuction as the main way to combat excess fat in the following cases:

  1. False acquired penis reduction syndrome, in other words hidden penis. It is the result of a “swelling” of fatty folds of the pubis in the area of the root of the penis and its visual shortening. A man turns to liposuction if diet and exercise fail.

Almost all obese men face this problem. This syndrome does not have any effect on erectile function and the ejaculation process, as well as on the quality of sperm. But in patients, a hidden penis can cause feelings of inferiority or psycho-emotional disorders, which negatively affects a man’s intimate life.

  1. Local deposition of fat in the pubic area. A man can put his figure in order through diet and exercise, but the pubic area is difficult to correct. Therefore, aesthetic considerations may also be a reason to consult a plastic surgeon.

Causes of prolapse and prolapse of genital organs

Prolapse develops due to weakening of the connective tissue, muscle corset and ligaments that form the pelvic floor.
Because of this, previously held structures begin to fall down. Vaginal birth and aging are major risk factors for the development of this unpleasant condition. Pelvic floor injuries occur against the background of complicated childbirth (perineal rupture, large fetus, multiple pregnancy, first delivery in adulthood). Often prolapse and loss of genital organs are caused by menopause, which triggers multiple changes in the female body. In addition to the above reasons, the following should be highlighted :

  • genetic predisposition, family factor;
  • excess body mass index (BMI > 30 kg/m2), obesity;
  • history of hysterectomy (removal of the uterus in the recent past);
  • hormonal imbalance (impaired estrogen synthesis);
  • constant lifting of weights, work with harmful factors;
  • bronchial asthma, constipation and other conditions that contribute to increased intra-abdominal pressure.

Since pathological changes are often found in other pelvic floor diseases, careful diagnosis of the disease is recommended to exclude more serious risks. Morbidity significantly changes the quality of life of patients, so they are forced to seek help from medical professionals.

Contraindications

All surgical interventions have their contraindications, and liposuction of the pubic area in men is no exception:

  • young age of the patient (up to 18 years);
  • a person's body weight is significantly higher than normal. Liposuction as a method of getting rid of excess fat is used when excess weight is no more than 20%;
  • diseases of the cardiovascular system, such as hypertension, hypotension, varicose veins, heart disease;
  • diabetes;
  • diseases of the blood coagulation system (hemophilia, von Willebrand disease and others);
  • stomach ulcer, ulcerative colitis;
  • oncological diseases;
  • hormonal disorders associated with malfunctions of the endocrine organs;
  • renal failure, nephritis and other pathologies of the excretory system.

Signs of genital prolapse

There are certain symptoms that indicate genital prolapse. These signs of prolapse depend on the type of disorder and the degree of weakening of the vaginal vault. In the early stages, there are no pronounced symptoms. Women don't even know that they suffer from prolapse. A doctor can notice abnormalities during a vaginal examination and taking a smear using a Papanicolaou test.

If the prolapse of the female genital organs is severe, many signs begin to be felt by the woman :

  • feeling of vaginal tightness;
  • sensation of a foreign body inside;
  • suspicious protrusion of the walls;
  • sexual disharmony in relationships with a partner (decreased sensation or pain during intimacy);
  • urination disorders (frequent trips to the toilet, “reservoir effect” syndrome, in which there is no sensation of complete emptying of the bladder, urinary incontinence);
  • difficulties during bowel movements (emptying the intestines from feces).

In case of violations, relapses of urinary tract infections are often observed. These symptoms may get worse towards the end of the day. Many women begin to feel better after they take a horizontal position and lie down on the bed. If the prolapse extends beyond the vagina, the organs become visible. Pain, bleeding, and ulcers may occur from constant contact with underwear.

Functions of the pelvic element

The structure includes red bone marrow, which is necessary for the normal process of hematopoiesis. All parts of the musculoskeletal system are capable of producing important body fluids. The pubic bone also has other functions:

  • supports motor activity;
  • helps keep the body upright without changing posture;
  • connects the upper part of the body with the lower part, ensuring uniform load on the limbs;
  • is responsible for the fact that a person runs, walks, and jumps normally;
  • protects the small and large intestines, as well as the genitals from injury.

Severe disturbances in the structure of the pubic bone lead to disruption of the functioning of the body.

Classification

In medical practice, there is a classification of the disease, when specialists indicate the degree of impairment. There are 4 main stages, according to which a treatment regimen for the disease :

  1. I degree - the cervix descends down to no more than 50% of the vagina;
  2. II degree - internal structures descend to the outer ring of the vagina;
  3. III degree - the walls of the vagina and the cervix come out and become visible from the side.
  4. At stage IV of the pathology, the body of the uterus also comes out through the vaginal vault. This condition is extremely difficult for a woman.

Patients with mild abnormalities describe a feeling of heaviness in the perineum. Some of them begin to feel pressure only when standing for long periods of time during the day or after doing physical exercise. In severe cases, patients report protruding organs that become visible to the naked eye.

Depending on where the pelvic floor is weakened, different complications may occur. Thus, weakness of the anterior vaginal wall leads to cystocele (prolapse of the bladder). An enterocele occurs when weakness affects the ceiling of the vaginal muscle vault. This provokes uterine prolapse. Defects in the posterior wall can cause rectocele (defecation disorder). Determining the exact location of the weakening plays a key role in prescribing treatment procedures.

Structure of the pubic bone

The pubis is a bony connection of 2 parts located at different angles in the pelvic region. Between them, the pubic symphysis is located perpendicularly - a semi-joint with strong ligaments, which can diverge by 0.5-1 cm. Forming the acetabulum, the pubic bone connects to the obturator passage, which is covered with thin membranous tissue.

In men, the bone has a sharp shape; in girls, it is more like a small cushion with a diameter of up to 1.5 cm. The promontory forms an elevation, which normally does not protrude much above the plane of the body, is located above the vagina and does not block the entrance to it. In men, the parts are connected to form a triangle located under the pubis.

Prevention: preventing genital prolapse in women is easier than treating it

It is much easier to prevent prolapse of the genital organs in women than to try to treat the pathology in all possible ways. Prevention is especially important for those women in whose family there have been cases of prolapse. Since prolapse occurs due to weak pelvic floor muscles, it is necessary to strengthen the muscular system with the help of special exercises. Physical activity in this area should be observed at any age.

Since training the vaginal muscles is a delicate moment, it is better to find a trainer who can assess the correctness of the training complexes. Today, many seminars, trainings and master classes are held on the art of mastering your own body. Here you can get an assessment of your capabilities. However, it is better to entrust your problem to a professional doctor at the Harmony clinic. The specialist will accurately point out the shortcomings and give valuable recommendations for improving intimate gymnastics techniques.

In addition to Kegel exercises, it is recommended :

  • try to avoid constipation (drink plenty of water, eat vegetables and fruits);
  • avoid lifting heavy objects;
  • control severe cough (take a course of treatment, quit smoking);
  • Avoid weight surges and weight gain.

If you think you are dealing with genital prolapse, there is no point in being embarrassed or isolating yourself from society. By visiting a specialist at the Harmony clinic, you will receive high-quality help, thanks to which you can get rid of the symptoms of a hated disease.

Exercises for men who want to remove fat from the intimate part of the body

When the warm-up is finished, the man should move on to the main part of the workout. Optimal exercises for burning pubic fat.

  1. Squats. There are a lot of options, which differ in the position of the legs, and can also be performed with a load. The squat works not only the pubic area, but also the muscles of the thighs, calves, and buttocks.
  2. Scissors exercise, which is performed while lying on your back by raising your legs 45 degrees from the floor and then crossing them. You need to do it quickly. It is worth performing the exercise as long as you have the strength to hold your legs. After this exercise, you can stroke your pubis with your hand to improve blood flow.
  3. Leg lift. This can be done lying on your back, hanging on parallel bars or a horizontal bar. The legs are raised 90 degrees very slowly and lowered in the same way. It is recommended to do at least three approaches.
  4. Bend backwards, forwards and to the sides while standing.
  5. While lying down, raise your upper body and reach for your toes with your hands.
  6. Torso twisting.
  7. Swing your legs in different directions.
  8. The plank is also suitable for men.

By performing these and other similar exercises, a man can not only get rid of pubic fat, but also improve his body in general. The main thing is to make training part of your life.

Genital prolapse - treatment options

Conservative treatment options include lifestyle changes, pelvic floor training, mechanical support of the vaginal vault using special devices, and pharmacological drugs. Such recommendations are given to women who are contraindicated for surgical treatment, as well as to those who do not want to undergo surgery or are preparing for it.

If conservative methods do not produce results, the doctor recommends treatment of genital prolapse with surgery. In this case, a detailed conversation is held with the patient, during which the emphasis is placed on the following points :

  • age and general health;
  • planning a future pregnancy;
  • desire to preserve vaginal function;
  • degree of prolapse, anatomical state of the pelvic organs.

It’s worth mentioning right away - the relapse rate after reconstructive surgery is 30%.
In surgical practice, two main approaches are used. Many surgeries are performed through the vagina. Laparoscopy is in demand - a procedure performed through narrow tubes inserted into small incisions. These procedures reduce scarring, minimize blood loss, shorten hospital stays, and speed recovery time. The doctor can correct the situation transvaginally, using laparoscopic techniques and robotic equipment. The purpose of the operation with any method is to correct prolapse of the genital organs, to carry out treatment to correct the anatomy, functions of the intestines, bladder and vagina.

The following techniques are used :

  • elimination of cystocele - surgery to restore a prolapsed bladder or urethra (urethrocele);
  • hysterectomy - complete removal of the uterus;
  • correction of rectocele - correction of prolapse of the rectum and small intestine (enterocele);
  • laparoscopy with restoration of the vaginal walls;
  • obliteration - complete closure of the vagina.

A surgical procedure called anterior colporrhaphy tightens the anterior vaginal walls, while posterior colporrhaphy tightens the posterior segment of the vagina. Laparoscopy is used to relieve stress urinary incontinence, hernias at the apex of the vagina, and to create support for walls weakened by hysterectomy. For this purpose, vaginal grafts made from synthetic and biological materials are widely used.

Transvaginal surgeries have an almost 100% success rate and are usually reserved for older patients with multiple medical problems. The vagina is shortened and sutured so that the pelvic organs no longer protrude through it. After these operations, sexual intercourse becomes impossible. Hospitalization is short (from 1 to 6 days). Upon discharge, patients are prescribed antibiotic therapy to prevent the development of infection and painkillers to relieve pain.
