How to choose the size and shape of a breast implant: the doctor explains

Despite the enormous possibilities of cosmetology and aesthetic surgery, their accessibility and popularity, not all celebrities use them. And although most celebrities give preference to preserving and enhancing their beauty and youth, there are also those who have decided to surrender to the power of time.

Dmitry Saratovtsev, plastic surgeon
Unfortunately, despite all the advances in medicine and cosmetology, new technologies and techniques, not all celebrities want to fight for their youth. Some do this absolutely deliberately, hoping that the face cream and mask will stop ptosis, reduce wrinkles and restore radiance to the skin. However, such miracles do not happen. Only doctors can help in preserving your beauty, especially at an advanced age. Here are just a few names of stars who should take care of their appearance.

Brigitte Bardot

In the 50-60s, this legendary Frenchwoman was talked about with aspiration: the actress was a real sex symbol all over the world, along with Sophia Loren. Therefore, for many, her earlier end to her film career came as a shock: the star left the profession at its peak, in the 70s of the last century. After this, Bardo stopped caring about her appearance and plunged headlong into human rights activities. Unfortunately, today the face of the performer of the role of Babette is furrowed with deep wrinkles and the skin is sagging. A total facelift and blepharoplasty could correct the situation.

Xive implants from Germany

When trying to understand which dental implants are best, you should not ignore reviews from dentists. Thus, most specialists in the field of implantology had quite positive impressions of using XiVE systems. Dentists who focus on the use of these implants have extensive advantages for the use of both the dental systems themselves and prosthetic elements. This ensures that effective treatment is likely to be provided in all cases.

At the same time, XiVE designs demonstrate excellent osseointegration rates (about 99%), which is confirmed by the results of active research work. Thanks to the use of German technologies, any errors are eliminated: proper contact of all components is ensured, which guarantees perfect aesthetics and impeccable functional characteristics.

The possibility of using counterfeit XiVE implants is also excluded, since each manufactured model is certified.

If we talk about the negative aspects when using the XiVE system, the key role here is played by the insignificant prevalence of products of this brand in dentistry in Russia. Moreover, with the traditional two-stage procedure, the osseointegration period of XiVE reaches approximately 90 days. This is quite a worthy parameter, since for other designs in the economy segment this figure can reach 6 months.

Israeli implants Alpha Bio

If you want to find the answer to which dental implants are better, it will be quite difficult to find negative reviews from dentists about the Alpha Bio system. First of all, this is due to the very good quality of manufactured dental implants. Secondly, the manufacturer pays close attention to its marketing direction and actively positions new product models on the market.

They are presented in a wide range, therefore, dentistry itself is focused on competently promoting these systems and generating impressive profits.

As for the negative aspects when using Alpha Bio, the above manufacturer does not have a sufficient selection of transition elements for fixing prosthetic devices. This suggests that, despite all the advantages of the system, if non-standard cases arise during the installation of orthopedic structures, the patient will have to choose between aesthetics and functional characteristics.

That is, focusing on the problem of which implants are best placed on chewing teeth, you can choose the Alpha Bio system. But such implants are preferable to be used in standard situations.

Dentium (South Korea)

The significant demand for Implantium dental systems in domestic dentistry is due to excellent reliability and efficiency indicators, as well as the affordable cost of the products. Considering the dilemma of which implant is best to place on chewing teeth, it can be argued that Dentium designs are quite suitable for recreating missing teeth in this section of the jaw. This is due to their good rate of osseointegration and good stabilization ability. However, Dentium implants cannot be used in unusual cases and in the process of implementing a one-step technique.

You should also take into account Dentium's extensive range of prosthetic components and adapters. Despite the excellent aesthetic and functional characteristics that can be recreated when using designs from this brand, they can hardly be compared with the Nobel Biocare and Straumann systems.

In addition, at the moment this company has little experience in the field of research activities and the use of modern technological solutions, which excludes the possibility of an extended warranty for fixed structures.

French Anthogyr implants

The 7th position in the ranking among the best dental implants is occupied by the Anthogyr dental system. First of all, this is due to the nuances of developing its design. In the manufacture of these structures, a planar connection with an adapter was used, which explains the appearance of a small distance between the components of the system. This may subsequently cause the mounting screw to loosen. Another “disadvantage” of such a system is the possibility of bone tissue resorption, which can occur after fixing an implant with a diameter of 5 mm.

Among the advantages of Antozhir it should be noted:

  • Probability of using titanium and zirconium adapters;
  • Shortened system fixation period;
  • Can be used to implement a one-shot protocol;
  • The likelihood of tissue injury is minimized;
  • Simplified implementation process;
  • The possibility of use is allowed in case of significant resorption of bone tissue;
  • Accelerated rates of osseointegration.

Osstem - the best budget implants

Fixation of Osstem systems is carried out similarly to other dental implants: the implantation of the structure is carried out using a traditional two-stage or one-stage method. However, you should be aware of the limitations that make it impossible to install Osstem implants.

If we turn to the advantages of using products from this brand, then, first of all, they relate to the process of prosthetics. In the manufacturer’s assortment you can find adapters and other orthopedic elements that can be used for a wide variety of prosthetic options: both when one unit is missing, and when it is necessary to recreate all the missing teeth.

Also, the “advantages” of using these Korean systems include:

  • Optimal ratio of quality and cost;
  • Extensive range of manufactured models;
  • A popular manufacturer whose products are used in many modern dentistry;
  • A good educational base for practicing specialists.

The actress herself refused the services of plastic surgeons, without trying to hide age-related changes. However, if previously they were able to be hidden, today the traces of time have left an imprint in the form of manifested ptosis of the upper eyelids. The performer of the role of Malena could benefit from blepharoplasty or endoscopic lifting of the upper part of the face. Thanks to these corrections, the star could eliminate crow's feet and raise her eyebrow line.

What breast implant fillers are there?

Breast implant fillers come in three types: saline, silicone, and biogel. Let's take a closer look:

  • The silicone filler is a special cohesive sticky gel. It has memory so it can stay in place if the shell ruptures. Silicone is safe for the female body and does not migrate after installation. Scientists have proven that silicone filler allows for maximum compliance with the anatomy of the breast. The gel has a lifetime guarantee and is the most common filler;
  • The saline filler is 0.9% sterile saline solution. The specialist injects the solution into the breast implants installed in the human body. Therefore, it becomes possible to form the required size and volume of the future breast. The skin gradually stretches after the operation and the solution does not allow the breast to maintain its shape for a long time. These implants require regular replacement, so girls choose them less often;
  • biogel is a natural polymer obtained from cellulose, identical to silicone. After the implant shell ruptures, the filler is absorbed and disintegrates in the tissues into components: glucose, water, carbon dioxide.

Each of the fillers has individual advantages and disadvantages. Biogel implants are considered the most dangerous, which can cause a number of negative consequences.

Catherine Deneuve

Source: @ yana_yanak
The legend of French cinema generally demonstrates good condition of appearance. At the same time, as she herself says, only injection techniques help her with this, including botulinum toxin injections. Most likely, this is the case, but the result could have been even better if the leading actress in “Beauty of the Day” had had her lower third of her face lifted, since Deneuve currently has a problem with her contour. The operation could eliminate jowls and improve the condition of the actress’s neck.

Where to choose?

Production of implants in Germany

Anatomical implants are recommended for patients who:

  • are distinguished by thinness;
  • have a narrow chest;
  • have a small volume of their breast tissue;
  • have pronounced mastoptosis with a small breast volume;
  • have breasts of different sizes.

Round implants are recommended:

  • young patients with well-developed mammary glands;
  • women who have sufficient volume and slight mastoptosis of the breast;
  • patients who want a more balanced breast shape.

Sarah Jessica Parker

Source: still from the series, @ djquintero
To the delight of fans of the legendary series “Sex and the City,” its creators were puzzled by the filming of its continuation. Famous actresses met again on the set a few years later to talk about how the lives of their New York girlfriends had changed. However, fans of the series have repeatedly expressed concern about the appearance of the main character Carrie Bradshaw. This is understandable - this style icon now looks different, with unpainted gray hair and wrinkles on her face. Parker herself is a supporter of natural aging and does not accept any interventions. However, deep folds on the face, a wrinkled network around the eyes and ptosis only add to the age of this still quite young actress. A good facelift could solve this problem.

Implants for breast enlargement

At the Health Factory MC Rostov-on-Don, only certified implants from well-known manufacturers are used for breast augmentation:

  • Brazil (Silimed)
  • Germany (Polytech)
  • USA (Mentor)
  • UK Nagor
  • France Arion

By following the links you can see the features of implants from each manufacturer, but there are also common features.

  • All implants used have certificates and permits for use in Russia, Europe and America.
  • Each implant is provided with a quality certificate, a warranty card, a unique number and a unique barcode.
  • All of the above-mentioned manufacturing companies use the latest developments in the production of implants and provide a lifetime safety guarantee .

Implant shape

Based on their shape, implants are divided into anatomical and round. The fundamental difference between round and anatomical implants is the fullness of the upper slope of the breast. Typically, round implants have a more convex top slope. Anatomical implants have a more diverse range of sizes in width, height and convexity (projection) with a “step” of 2-3 mm. Thanks to this, breasts enlarged with the help of anatomical implants often look more natural. Currently, anatomically shaped implants with a round upper pole have appeared, which allows you to ideally select the required width, height and convexity (projection) to obtain a natural result, and at the same time give a beautiful neckline shape, as if round implants were used.

Anatomical implant with round upper pole:

Any company that manufactures implants offers an extensive line (200-300 sizes) of both round and anatomical implants, with increments of several millimeters. allowing the plastic surgeon to select the ideal implant parameters. There is no point in thinking about which round or anatomical ones are better - you need to try them on during a consultation to determine which option you like best in the mirror.

Properties of implants

The properties (softness, elasticity, mobility, probability of displacement and inversion, probability of contracture formation, time of natural wear) of breast augmentation implants are determined by two parameters - the properties of the filler and the properties of the implant shell.

Filler

All modern implants are filled with medical silicone. Implants filled with saline, petroleum jelly, and other substances are a thing of the past. Medical silicone for implants differs in the degree of cohesiveness (density). For round implants, both soft (flowable) silicone and medium- and high-hesive (non-flowable) silicone can be used. The less cohesive the silicone (more fluid), the more likely the formation of waves on the surface of the implants (and breasts enlarged with the help of such implants).

Most anatomical implants for breast enlargement are filled with a highly cohesive gel that does not flow even if the implant is damaged:

Implant shell.

The shell of breast enlargement implants must be strong, elastic, and prevent silicone from leaking beyond the implant.

Modern implants have a multilayer shell that prevents the gel from leaking beyond the implant. To be certified, a breast implant must withstand a pressure of 300 kg per 1 sq.cm.

Before being sent to plastic surgery clinics, all implants are thoroughly tested for shell strength. Here is one of the standard tests:

It is unlikely that implants will experience such compression in life.

All implant shells are divided into

  • Smooth,
  • Rough (Textured, macro-, micro-textured)
  • Polyurethane coating

The presence of a roughness or polyurethane coating on the surface of the implant helps it to be fixed to the tissue and prevents it from moving, turning over and the formation of capsular contracture (compression of the implant by the capsule formed around it)

  • Smooth implants are not fixed to the tissue in any way; they are held in place only by the capsule that forms around them. Smooth implants have the highest risk of displacement, inversion (which is why they are not anatomical) and capsular contracture. Smooth implants are currently practically not used for breast augmentation.
  • Textured (macro-, micro-textured) have a rough surface. When using such implants, it is assumed that the capsule formed around the implant will be fixed to the implant shell due to its roughness. But, unfortunately, the capsule that forms around the textured implant is not always fixed to the implant. In such cases, the implant may move towards the armpits and downwards, and contracture (compression) of the capsule around the implant may also occur.
  • The polyurethane coating on the surface of the implant immediately and reliably fixes the implant to the surrounding tissues, preventing it from moving or turning over. A separate bonus of the polyurethane coating is the prevention of capsular contracture due to the formation of a capsule with a particularly soft, foam-like structure.

The most modern implants currently are polyurethane-coated implants.

Implants with polyurethane coating have been produced since 1989, i.e. This is still a relatively new technology. And like everything new in surgery, it will take many years before polyurethane-coated implants occupy their niche.

Initially, polyurethane-coated implants appeared as a way to solve the problem of contracture and rotation (turning implants), and were used only for repeated operations, but over time, plastic surgeons began to notice other positive qualities of polyurethane-coated implants and use them immediately, without waiting for problems after installation of implants of previous generations.

The main difference between polyurethane-coated implants is the structure of their shell, which ensures reliable fixation of the implant and due to this:

  • Implants with polyurethane coating are installed immediately as needed and there is no need to wait several months for them to fall into their natural position.
  • Polyurethane-coated implants cannot move down or to the sides
  • Polyurethane-coated implants do not turn over
  • Polyurethane-coated implants help cope with congenital deformities. For example, straighten asymmetrical, crooked or upward folds under the breasts and securely fix them in a new position.
  • Polyurethane-coated implants help eliminate mild ptosis (sagging) and sagging breast tissue without unnecessary scarring.
  • Polyurethane coating reduces the risk of seromas (fluid accumulations) around the implant both in the early postoperative period and several years later
  • Implants with polyurethane coating can be installed without damaging the pectoralis major muscles, thanks to this they do not interfere with active sports and do not limit the load on the arms
  • Polyurethane-coated implants do not interfere with breastfeeding
  • Rejection of polyurethane-coated implants is much less common than textured and smooth implants
  • Ripping (waves visible through the skin on the surface of the implant) is less pronounced and less common with polyurethane implants
  • Polyurethane coating prevents the formation of a dense, hard capsule around the implant (contracture)

Due to the quick and good fixation of polyurethane-coated implants, it turned out that it was possible to solve the problem of displacement of implants laterally (to the side and towards the armpit), downwards (on the stomach), and to eliminate synmastia (displacement of implants towards each other with the formation of a common cavity). Polyurethane implants allow for good (and most importantly reliable!) positioning of the submammary fold if it is necessary to move it, more clearly give the shape of the breast with various deformations, and compensate for moderate breast ptosis without additional scars. Many surgeons report a lower percentage of gray (fluid accumulation around the implant) in the postoperative period when installing polyurethane implants. Polyurethane implants have a greater chance of engraftment if there has already been polyacrylamide gel (it is impossible to remove polyacrylamide gel 100%) or rejection of silicone implants. Breasts with installed polyurethane implants are less prone to animated deformations (deformation and displacement to the sides when the pectoralis major muscles are tense), which makes polyurethane-coated implants more preferable for women actively involved in sports. The slightly different consistency of polyurethane implants reduces the frequency and severity of ripping (the visibility of waves on the surface of the implant through the skin in places where the thickness of the soft tissue above the implant is insufficient).

Currently, polyurethane-coated implants are increasingly being used not only for cases of implant replacement due to problems, but also in complex cases, cases of increased risk of problems, and even simply for prevention in relation to the occurrence of the problems described above.

The disadvantages of using polyurethane implants include the greater technical complexity of their installation. They need to be installed immediately as needed. Of course, polyurethane implants have no place in the hands of novice surgeons. The incision for installing an implant with a polyurethane coating is on average 0.5 cm larger than for a classic implant with a textured coating. It is extremely difficult to place a polyurethane implant from the axillary approach. The slightly different consistency and size range of polyurethane implants also adds complexity to surgeons who are accustomed to working with classic implants. And, of course, the price... The purchase price for clinics for polyurethane implants is approximately 2 times higher than for classic ones, which affects the cost of the operation, but is compensated by an order of magnitude reduction in the most common risks of breast augmentation with implants. A significant reduction in the number of problems after breast augmentation when using polyurethane implants compensates for their higher purchase cost, and allows you to keep the price of breast augmentation within an acceptable range.

So, what is a “polyurethane implant?” This is practically the same silicone implant, only in a different polyurethane shell, which during surgery is fixed to its own tissues and does not allow the implant to move/turn over and programs the formation of a different capsule structure around the implant, less prone to contracture. After several years, the polyurethane shell of the implant dissolves and the same silicone implant remains, but in a capsule with a different structure.

Implants with polyurethane coating are produced in the world by only two companies (Polytech and Silimed), which were originally one company Polytech - Silimed, but split up.

Polytech's production and headquarters are located in Dieburg, Germany.

The Silimed campaign is located in Rio de Janeiro, Brazil.

The lines of polyurethane implants from both companies have some differences in shape, size range and consistency, as well as in terms of warranty programs.

The ranges of sizes and shapes of both companies are very diverse. The differences in the shape of round implants from Polytech and Silimed are negligible. Differences in the anatomical shape are more significant; the differences are mainly in the shape of the upper slopes (the part of the implant that forms the décolleté area). It's up to who likes what more. With anatomical implants from Polytech, this part goes more in a straight line, creating a classic straight line of the neckline.

Anatomical implants from Silimed have forms of anatomical implants with a straight and even slightly concave upper slope (Natural and Enhance), but the Nuance form deserves special attention - anatomical implants with a convex upper slope, allowing you to create the effect of a full neckline.

At a consultation with a plastic surgeon at the Health Factory MC, Rostov-on-Don, you will be able to see and discuss the advantages of each type of implant, try on different sizes and shapes of implants for breast enlargement and choose Your size and Your shape of the implant.

Often, patients try to independently determine the shape and size of the implant from photographs before and after breast augmentation presented on websites. Every breast is unique. Wishes are also individual. Some people want to enlarge their breasts a little, while others want the largest possible breast size. One dreams of a chic neckline with convex breast domes, and the other for a natural shape. Taking into account the initial data and wishes, the shape, size and degree of cohesiveness (elasticity) of the implant are selected. Modern implants have 3 degrees of cohesiveness (elasticity) and about 200 options for size and shape. Choosing an implant is much more complicated than “second or third size.” Leave the question of choosing an implant to a plastic surgeon. When looking at the photos before and after breast augmentation, try to decide only whether your idea of a beautiful breast coincides with the idea of the beauty of your plastic surgeon. Describe your dream to the plastic surgeon in as much detail as possible and trust the professional. An experienced plastic surgeon will help your dream take on real shape!

Cynthia Nixon

Source: still from the series, @cynthiaenixon
Another actress from the series beloved by millions, Cynthia Nixon, would also do well to turn to plastic surgery specialists. After all, the performer of the role of Miranda, like Parker, refused in principle from any manipulation. Today Nixon is only 54 years old, but she looks much older. Therefore, she could be recommended treatments that were more effective than traditional care. With the help of blepharoplasty, she could get rid of hernias on the lower eyelids and make the eyebrow line much higher, thereby rejuvenating herself by 5-10 years.

Round implants and their features

Round implants, as the name suggests, have a round shape. Their point of maximum projection is located under the center of the base. In this case, the projection size may vary.

With mammoplasty, it is possible to enlarge the mammary glands by several sizes at once and obtain maximum volume. And although the breasts after surgery do not always look natural, this is compensated by a high aesthetic effect and visual appeal.

This is especially noticeable in the before and after photos, where the difference between the initial appearance of the mammary glands and the postoperative one is clearly visible.

Round implants make the breasts fuller, firmer and allow you to achieve a “push-up” effect.

When is it appropriate to use

Round breast implants are rarely used in surgeries for medical reasons. As a rule, they are used in aesthetic surgery to obtain volume, when it is necessary to enlarge the mammary glands by several sizes at once. Allows you to enlarge the upper pole of the mammary gland.

It makes sense to install them when the breast shape is round, there is a sufficient amount of glandular tissue, or when the natural appearance is not important and volume is what is required. However, when manufacturing implants, material is of paramount importance. Their naturalness, both visual and tactile, depends on it.

Most often, endoprostheses with a medium or high profile are used in augmentation mammoplasty. However, the specific choice of form depends on the physiological build of the patient and the structure of her mammary glands.
