All people are subject to the aging process, and it cannot be stopped. However, it is within our power to try to maintain the internal health and external beauty of the body. Modern cosmetology allows us to preserve and maintain the health of our skin. For this, a huge arsenal of hardware and non-hardware techniques is used. One of the leading places in it rightfully belongs to the procedure of fractional photothermolysis.

What is fractional photothermolysis?
Fractional photothermolysis technology was discovered in 2004 and consists of the following: a laser beam is distributed into many small microbeams. Each of them is several microns in diameter, and they are located at some distance from each other
Fractional photothermolysis procedure:
| Each microbeam, depending on the parameters used, can penetrate the skin to a depth of up to 2 mm, the skin tissue in this place is heated to 60°C, and proteins are denatured. As a result of the procedure, “columns of microdamage” are formed in the skin throughout the treated area. These microcolumns trigger reparative processes, activate the synthesis of collagen and elastin fibers, and increase skin turgor and elasticity. |

How is the photothermolysis procedure performed?
- The fractional photothermolysis procedure is easily tolerated and does not require long-term rehabilitation.
- Thanks to the small diameter of the microcolumns, the basal layer of the epidermis is restored within 24 hours, and skin redness disappears within a few days.
- Laser fractional photothermolysis has virtually no restrictions or contraindications.
- You can treat any part of the body, even the most delicate ones - eyelids, neck, chest, inner thighs.
To obtain maximum results, a course of 3-4 procedures is recommended with an interval of 1 month. The effect is potentiated with each subsequent procedure and persists for several years.

Mechanism of action
Fractional laser photothermolysis causes mini-injuries - thermal burns, which form a microscopic treatment zone in the thickness of the dermis.
The goal is to stimulate the regeneration process in the area of problematic skin - wrinkles, stretch marks, age spots, scars, scars, etc. Thousands of such mini-injuries ranging in size from 0.1 to 0.4 mm trigger the process of stimulating skin restoration with the production of collagen, elastin and hyaluronic acid, which are the basis of connective, epithelial and nervous tissues.
The regeneration process stimulated in this way is a true rejuvenation of the skin of the face, eyelids, décolleté and other problem areas.
Stages of implementation
Before starting the process, the cosmetologist talks with the patient. During the conversation, it becomes clear that there are no contraindications for using the Fraxil technique. Further actions take place according to a standard protocol, including a set of measures:
- Makeup removal - cleansing the skin of all types of cosmetics using a neutral gel and soothing lotion.
- Treatment of the intended areas of laser irradiation with an anesthetic based on lidocaine (40 - 60 minutes before the start of the procedure). In some cases, instead of an anesthetic, it is possible to take an anesthetic drug (Ketonal).
- Removing the anesthetic cream with soft wipes and treating the area with a special sliding lubricant.
- Grinding process. The duration of this stage depends on the amount of work (from 15 to 50 minutes).
- After the procedure is completed, the skin is cleansed of lubricant.
- A soothing cosmetic is applied to the treated areas to reduce redness and swelling.
- During operation of the laser installation, the built-in cooling system is turned on. This allows you to protect the skin from overheating and avoid complications. The number of sessions and visit schedule for you will be determined by the doctor individually (on average 2 - 4 procedures every 21 - 30 days).
Attention! Be sure to tell your cosmetologist if you have an allergic reaction to medications containing lidocaine.
Skin care after the procedure
Fractional photothermolysis using a CO2 laser is a rather traumatic procedure that is incompatible with periods of solar activity due to the high risk of hyperpigmentation. The ideal time for the procedure is from October to March.
Due to increased photosensitivity of the skin after the procedure, you should avoid contact with the sun's rays and be sure to use sunscreen with SPF 30-50 before going outside. For the entire rehabilitation period, it is prohibited to visit the swimming pool, bathhouse and sauna. The doctor will also prescribe cosmetics and ointments that will speed up the recovery process.
Advantages and effectiveness of fractional laser
The fundamental advantages of the Fraxel laser over other hardware methods of rejuvenation are that the outcome of treatment is always predictable: the doctor knows in advance what effect will be achieved as a result of therapy. In addition, the device is simple and safe to use. With the help of Fraxel, a deep restructuring of the collagen framework of the dermis occurs. The equipment can be used for any area of the body. Skin color type does not affect the result.
The fraxel laser is able to solve most aesthetic problems of the dermis, regardless of their severity, age and skin type of the patient. The result of the treatment is noticeable a few days after the first session: the skin is evened out and acquires a healthy color. A course of procedures allows you to achieve a lasting, long-lasting effect. After 4 - 5 consecutive visits to the laser therapy room, patients look 8 - 10 years younger. Your skin will retain its newly acquired beauty and youth for several years.
Complication after fractional laser rejuvenation - “gauze effect”
Over the past 10 years, laser technologies have become very popular among patients in the correction of age-related changes. There are many cosmetology devices for laser skin rejuvenation, as well as beauty salons and clinics where you can undergo this procedure. And, as a result, patient reviews were divided. Some became 5-7 years younger and were very pleased with the procedure, while some patients received something completely different from what they expected. The main complications during fractional laser rejuvenation include the occurrence of a “gauze effect” and the appearance of foci of hyperpigmentation.
The “gauze effect” is a fairly common complication of fractional laser rejuvenation and is a network of atrophic scars on the surface of the skin of the face and body. It usually occurs when using a fractional CO2 laser, when the laser beam passes into the skin in the form of a uniform grid of microbeams.
The main reason for the “gauze effect” is associated with insufficient experience of the specialist who performed the procedure. The cosmetologist was unable to correctly determine the thickness of the epidermis and dermis, did not take into account all the individual characteristics of the patient’s skin and, accordingly, incorrectly set the procedure parameters. The procedure parameters can vary significantly not only between different people, but also when treating different areas in one patient, for example, when correcting wrinkles in the eyelid area and pouch above the upper lip. Therefore, a consultation with a cosmetologist is an important step before the fractional laser skin rejuvenation procedure. The doctor is obliged to pay due attention to the patient’s cosmetic history and prescribe medications for post-procedure care in order to neutralize the risks of the formation of small atrophic scars - the “gauze effect”.
If the network of scars after fractional laser rejuvenation on the face lasts more than 2 weeks, it is necessary to take urgent action to eliminate it. Otherwise, correction of this complication may take many months. Patients do not want to solve problems with their attending physician and turn to other cosmetology clinics. This complication should be treated by a qualified cosmetologist in a clinic setting.
The main methods for correcting the network of atrophic scars after fractional photothermolysis are:
- Laser peeling Pearl on the Cutera device
- Laser rejuvenation Genesis using the Cutera device
- Biorevitalization with combined drugs
The choice of treatment method depends on the aesthetic condition of the patient’s skin, age, cosmetic history, as well as the time of year.
Pearl laser peeling with a wavelength of 2790 nm is a modern effective correction method. The laser beam passes into the skin as a continuous spot, and not in the form of a grid as with fractional laser rejuvenation. This safe and controlled procedure restores radiance and radiance to the skin. The predictability of Pearl laser peeling sets it apart from chemical peels, especially medium peels, which are often used to eliminate the “gauze effect.” The procedure can be performed on any area of the skin, including the thin skin of the upper and lower eyelids, but laser peeling has seasonal restrictions.
In the summer, in our Aesthetic Medicine Center, good results in the treatment of the “gauze effect” were obtained by combining Genesis laser rejuvenation on the latest generation Cutera device and biorevitalization with combined preparations containing, in addition to hyaluronic acid, amino acids, vitamins, growth factors and other components necessary for skin restoration. For patients over 38-40 years of age, the drug of choice is Meso - Wharton P199 / Mezovarton.
Genesis laser rejuvenation can be used on patients at any time of the year with any skin type. This is a non-invasive procedure, i.e. It is carried out without violating the integrity of the skin; there is no rehabilitation period. Genesis laser rejuvenation is well tolerated by all patients; during the procedure they only feel heat, no anesthesia is required. After the procedure, minor hyperemia goes away on its own within an hour. The effectiveness of Genesis laser rejuvenation is due to the impact of a microsecond laser on the dermis, which leads to the activation of fibroblasts, improved microcirculation and reduction of collagen fibers. Clinically, this is expressed in improving skin texture, narrowing pores, smoothing wrinkles, and eliminating the network of atrophic scars, including the “gauze effect.” The procedure is performed on any area of the skin of the face and body, except for the eyelids.
The number of procedures and their frequency are determined by a cosmetologist during a consultation.
Result: what will thermolysis please you with?
The effect of the procedure should be assessed a month after skin treatment. Thanks to the pulse effect on the skin, you can achieve the following results:
- correction of scars - post-acne, atrophic and hypertrophic, stretch marks, traumatic;
- narrowing pores and increasing skin smoothness;
- evens out facial skin tone;
- improvement of relief due to the disappearance of fine wrinkles and smoothing of deep folds.
This procedure should only be carried out by an experienced cosmetologist using a certified device in order to reduce all risks of side effects to zero. We are ready to help our patients every day from 9:00 to 21:00!
