Facelift: endoscopic face lifting, smas lifting

Indications for face lift

With age, a person's face changes, excess skin appears, and the surface area of the face increases. The appearance of a wide “floating” face appears. A decrease in the elasticity and firmness of facial skin contributes to the deterioration of appearance. The soft tissues of the face droop, furrows and folds are exposed, and ugly reliefs and contours appear. During the consultation, patients show how they want to look. To do this, they lift their cheeks, temples, eyebrows up, tighten the oval of their face, momentarily regaining their youthful contours. This indicates direct indications for a facelift. No one better than the patient himself can determine the indications for facelift surgery, because the psychological attitude towards the operation and desire are very important. You need to understand: a facelift is the most effective anti-aging method of all, including conservative methods, minimally invasive procedures, lasers, etc. Only a full-fledged face lift can radically improve your appearance for a significant period (about 10 - 15 years).

Contraindications

The patient's age is not a decisive factor in refusal of surgery or indication for it. We look more at the patient’s health status, the presence or absence of chronic diseases. A facelift is usually performed on patients between 40 and 65 years of age. This is explained by the fact that in a younger state it is possible to do without surgery, but in an older age, surgery can lead to negative consequences, both for the patient’s health and for healing. At this age, skin regeneration decreases significantly and the rehabilitation period increases.

Operation

A facelift operation may involve not only skin tightening, but also a complex operation involving tightening of the deep muscular-fascial structures of the cheeks and neck. For this purpose, SMAS plastic surgery of the cheeks and neck is performed. According to indications, plastic surgery of the cervical muscle (platysma) is performed, in which the vertical chords of the diverged edges of the platysma are eliminated. Access is through a small 2.5 cm incision in the mental sulcus.

Simultaneously with a face lift, a forehead and eyebrow lift can be performed according to indications. Forehead and eyebrow plastic surgery can be performed using endoscopic video technology or in an open manner (“coronary access”). In the case of an endoscopic forehead and eyebrow lift, the operation is performed through minimal skin incisions. Endoscopic surgery is more indicated for young people with small excess forehead skin without deep wrinkles, with a low hairline on the forehead. If there is significant excess skin on the forehead, surgery with the so-called “open” access is more recommended - coronary forehead lift. With the open method, fairly large excess skin of the forehead (3-4 cm) is removed, wrinkles are straightened, eyebrows and glabella are raised, and drooping outer corners of the eyes are raised.

A facelift is a surgical intervention; to prepare for such an operation, it is necessary to undergo tests and undergo additional examinations, which should exclude contraindications to plastic surgery. The operation takes up to 2 hours when working with facial skin, up to 4 hours when lifting the skin of the neck, face as a whole, up to 6 hours when lifting the forehead and eyebrows.

Rehabilitation period

The rehabilitation period is divided into early postoperative and late. Bruising and swelling are typical symptoms for several days after surgery. Sutures are removed 5-7-12 days after surgery, depending on the location of their application and the speed of healing in a particular patient. The early postoperative period lasts 2 - 3 weeks. At this time, swelling goes down, bruises resolve, and postoperative scars form. During this period, it is very important to follow and follow all the surgeon’s recommendations so as not to spoil the result of the operation. The help of a cosmetologist during this period is also important. The preliminary effect of a facelift can be observed 3-4 weeks after surgery. The result can be finally assessed only 6-12 months after the operation, when subcutaneous and dermal scars are fully formed, facial expressions, blood circulation and lymphatic drainage are fully restored.

The goals of cosmetic facial surgery are:

  • restoration of a youthful appearance of the face without loss of recognition.
  • achieving harmonious features, proportions, volumes characteristic of a young face.
  • help the patient look younger than his age and his peers.
  • in essence, to give back to the patient what they had and what time, gravity, the environment slowly but surely took from them.

What operations do we perform for facial rejuvenation?

3S-lifting – deep and detailed SMAS lifting of all areas of the face without the use of long incisions.

Patients who plan complete, consistent and comprehensive rejuvenation resort to such facial surgery. If you want to get rid of all signs of aging and aesthetic problems, without limiting yourself to a local lift of one or two areas of the face, 3S-lifting is exactly what you need.

3S-Lifting solves the following problems:

  • Neck lift. Eliminates excess fatty tissue and skin, cords and wrinkles that indicate age.
  • Restoration of a clear contour of the CHIN AND LOWER JAW. Eliminates double chin, restores the graceful contour of a youthful face and a clear “angle of youth” (the right angle between the line of the neck and chin).
  • Eliminates jowls, nasolabial folds, returns a beautiful jaw line and V-shaped oval, characteristic of a young face. Lifts drooping corners of the lips and eliminates marionette lines around the mouth
  • Corrects the central area of the face, including the painter's area.
  • Restores youth to the upper area of the face and improves the eyebrow line.

3S-lifting is based on three main principles that guarantee you a beautiful, safe and very long-term result.

Facelift, lifting.

The facelift operation is aimed at eliminating sagging cheek tissues, restoring the correct oval line, and lifting the chin with the formation of the chin-cervical angle. It can be combined with liposuction (fat removal) of the chin and neck, oval line of the face and cheeks for full faces. During the lifting operation, the fascia and muscles of the face are lifted, and excess skin is sparingly excised. With this approach to SMAS-lift, all tension forces are applied to the deep structures of the face, and not to the skin. As a result, the seams are delicate and invisible. The face gains a youthful appearance without distortion of features, the reliefs and contours that the patient had in his youth are restored. Healing after lifting occurs within 2-3 weeks. All sutures are removed 5-12 days after surgery.

Mendelssohn lifting (space lifting)

Are you thinking about a face lift, but doubt whether the results will look natural? Doctors at the Pirogov Clinic perform facelifts using a unique technique that is in demand all over the world. This is Mendelssohn lifting, or spacelifting (from the English space - space, to lift - to lift).

What is spacelifting?

The method was first named after its author, Australian professor Brian Mendelson. The technique is based on deep research into anatomy and requires masterly execution. In the tissue structures of our face there are spaces (spaces) located under individual groups of facial muscles. With age or due to individual characteristics, the spaces increase, which gives the effect of sagging of the muscle frame. The Mendelssohn lift differs from the classic SMAS lift in that the surgeon acts in these voids during the operation, restoring their previous condition. Thus, it does not need to affect all tissues, but only individual areas, which makes the technique the most gentle and allows one to achieve a rejuvenation effect for 10–15 years, without losing natural facial features.

Who is the Mendelssohn lift suitable for?

Spacelifting is gaining more and more popularity not only among those who are concerned about obvious age-related changes on the face, but also among those who have just begun to experience them or who want to make the oval of their face clearer. Thus, the intervention has no age restrictions.

Mendelssohn lift allows you to:

  • eliminate gravitational changes such as jowls,
  • get rid of nasolabial folds, “puppet wrinkles”,
  • restore a clear oval face,
  • remove double chin
  • correct expression wrinkles,
  • achieve a rejuvenation effect for 10 – 15 years.

How is the operation performed?

The Mendelssohn lift technology involves minimal tissue trauma. The surgeon performs all manipulations through two small accesses near the ear area. Lifting lasts 2–3 hours and is performed under general anesthesia. After the operation, the patient spends one day in a comfortable room at our clinic under the supervision of specialists.

Recovery period

The recovery period after the Mendelssohn lift is minimal. After the patient spends one day in the hospital, he can return to his normal life within a week after the operation, following all the doctor’s recommendations. The sutures are removed on the sixth day, leaving no visible traces of the intervention.

Mendelssohn lift result

The Mendelssohn lift affects deeper layers than a classic lift, which means you will look 10 to 15 years younger and this effect will last up to 15 years. After the operation, the oval of the face is restored, obvious signs of age-related changes disappear, the skin is tightened, and the lines of the cheekbones are outlined.

Spacelift price

The cost of surgery in our clinic is from 195,300 rubles. The exact cost will depend on the desired result and will be clarified during a consultation with a plastic surgeon.

Neck plastic surgery.

Aimed at restoring a youthful contour in the neck, chin, and cervical-mental angle - it becomes straighter and clearer. Vertical cords on the neck, which are formed as a result of loss of tone of the cervical platysma muscle and divergence of its edges, are also eliminated. During the operation, excess fat on the chin is removed, the muscle is repositioned and its edges are sutured. The result is a smooth neck, a tightened chin, and an improved facial contour. Not accompanied by skin excision. The seam is located in the chin fold and is only 2 cm long.

Forehead lift.

The purpose of the operation is to rejuvenate the upper third of the face. Aimed at eliminating forehead wrinkles, vertical wrinkles between the eyebrows, smoothing out wrinkles in the outer corners of the eyes, lifting drooping eyebrows and giving them the correct shape. Operation options:

  • Open method - a so-called “coronal” incision is made, which can run along the hairline or 4-8 cm behind the hairline (depending on the height of the forehead). Indicated for patients with excess skin on the forehead, with drooping eyebrows below the upper edge of the orbit, with a high hairline, when moving the growth line posteriorly to the parietal region is undesirable.
  • Temporal lifting – localization of incisions in the temporal areas from 3 cm to 5 cm on each side. During this operation, the outer third of the eyebrow is tightened and the “crow’s feet” are straightened. This access can also be used to lift the middle third of the face. To correct glabellar wrinkles and lift the brow heads, a 1.5 cm incision is made in the midline behind the hairline.

  • The closed endoscopic method has several advantages over open methods. Allows you to raise the tissues of the forehead and eyebrows through 1-2 cm incisions in the scalp without increasing the height of the forehead. The use of an endoscope helps to more accurately treat the muscles of the forehead and between the eyebrows, without injuring the sensory nerves in this area. The intervention time is shortened, there is less trauma and blood loss, and as a result, the recovery period is faster. It can be successfully used in young patients with initial minor age-related changes; it can improve facial aesthetics at a young age, without traces of surgery. Also indicated for sparse hair on the head and in men.

Three principles – 3 capital “S”:

  • S hort scar (short cut);
  • S afety SMAS (safe SMAS lifting);
  • S support thread system (an innovative system for supporting and fixing tissues).

Plastic surgery does not stand still, and today many specialists in the world and in Russia promise their patients a radical rejuvenation effect with the help of a facelift. Why can the method of facelift with a shortened incision, created by Professor Bely, be called the best method of anti-aging facial plastic surgery?

  • Firstly, this is the most complete lift of all areas of the face . While with traditional surgical SMAS lifting only the peripheral area is lifted, 3S-lifting provides full lifting and SMAS plication, including tissue lifting in the middle zone. This allows you to COMPLETELY get rid of nasolabial folds and lift the drooping corners of your lips. While with a classic lift it is impossible to completely eliminate these problems.
  • Secondly, of course, this is the famous short cut technique , which brought Igor Bely world fame. The unique 3S-lifting facelift technology uses a shorter incision (usually 2-3 times shorter than other surgeons). In this case, the hair growth line does not change, unlike classical techniques. After completion of rehabilitation, the suture is difficult to find, sometimes even for the patient himself.

A short stitch means not only the absence of visible traces of the operation, but also less blood loss, shorter and more comfortable rehabilitation.

  • Thirdly, exceptional safety of the operation . The 3S facelift is performed in the deep layers of the face, under the tissues where important blood vessels and nerves lie. This means there is no risk of damaging them. Few doctors warn their patients that the facial nerve may be damaged during a classic circumferential facelift. Thanks to the 3S-lifting technique, the risk of damage to nerves and blood vessels is completely eliminated.
  • Fourth and finally, this is the long-term outcome . When deciding to undergo a SMAS facelift, patients have the right to expect long-term results and no need for repeat surgery. It is this effect that is ensured by the author's patented Support thread system, which ensures reliable fixation and support of tissues in their new position.

3S-lifting will give you back your youthful face and preserve it for you and your loved ones for many years and decades. Sign up for a consultation and in 2-3 weeks your dream of becoming the youngest and most beautiful wife, mother, grandmother, and maybe even great-grandmother will come true!

Midface lift

It is aimed at repositioning the facial structures that define the middle third of the face: cheeks, nasolabial folds, lacrimal-zygomatic groove, drooping malar fat. When lifting the middle zone of the face, the face acquires lost harmony, contours and reliefs are restored, folds are smoothed out, and takes on a shape close to an inverted triangle with high, full cheekbones. The midface lift operation can be combined with injection transplantation of your own fat using the lipofilling method. Incisions are made in the area of the temples of the scalp, in the wrinkle in the area of the outer corner of the eye 1 cm. Often performed with endoscopic lifting (lifting) of the forehead

Clients for cosmetic surgery for a midface lift can be women and men, regardless of age. Before the operation, a mandatory examination is carried out: blood tests, ECG, X-ray of the lungs. It is recommended to stop smoking 2 weeks before the procedure and stop drinking alcohol 3 days before. It is prohibited to take any dietary supplements (biologically active food supplements) 2 weeks before surgery.

Disadvantages of SMAS (SMAS) lifting

How is a lift done using the SMAS method?

Extensive skin detachment and an equally extensive SMAS lift are carried out through a long incision that passes in front of the ear, goes around it and behind it extends quite far into the scalp - naturally, with hair loss at the incision site and in the occipital region.

Each surgeon has his own method of making the sutures unnoticeable, but it is precisely because of the long suture that the classic SMAS lift is called “circular”.

By lifting and fixing tissue, SMAS creates a kind of corset under the skin, but does not take into account the uneven aging of the face. Therefore, the face often looks unnaturally stretched.

And, of course, the big disadvantage of lifting using the SMAS method is the long recovery period and the risks associated with a large intervention area .

Despite the generally accepted SMAS lifting technique, each surgeon has his own technique for influencing subcutaneous structures. Therefore, the duration of the effect is different.

The patient will never know exactly how the surgeon dealt with the deep structures of the face during the operation. One doctor will lift the tissue widely and firmly fix it, while another may limit himself to minimal impact. Having made the postoperative period easier for the patient, such a surgeon himself does not strain himself much, as if “eliminating” possible risks. And although this is not quackery, but the author’s understanding of the task at hand, it often runs counter to the interests of the patient.

Against this background, space plastic technology stands out favorably. This technique of facial plastic surgery eliminates any variations in the impact in relation to deep structures; This is a standardized technique, which at the same time takes into account the individuality of each person.

For this reason, among other reasons, space plastic surgery according to Brian Mendelson is considered a revolutionary breakthrough in facial surgery.
