Possible complications

— Biorevitalization can be classified as a classic procedure in aesthetic cosmetology. It is prescribed to many people. And yet: at what age and what category of patients is it necessary?
— By biorevitalization we mean intradermal, very superficial injections of unstabilized hyaluronic acid. It is introduced to moisturize the skin. Figuratively speaking, hyaluronic acid is for our cells like water for a jellyfish. We create a comfortable environment in which it will be easier for cells to exchange information, carry out nutrition processes, etc. Therefore, the procedure can be prescribed at least from the age of 20. It is especially indicated for people with dry, dehydrated skin, those who like to go to the solarium and sunbathe a lot. It would be a good idea to take the course on the eve of a trip to hot countries. Biorevitalization is also necessary for signs of photoaging, pigmentation, and age-related changes in the fine-wrinkled type of aging. And, of course, we recommend doing it before laser resurfacing or 2-3 weeks after it, when the scabs come off. This will speed up the process of tissue regeneration and improve skin condition.
— I’ll ask a question that worries many: why do small pimples remain on the surface of the skin after the introduction of biorevitalizant?
— We inject a preparation of unstabilized hyaluronic acid into the dermis using the papular technique. Small peas form on the surface - this is a “depot” of hyaluronic acid. It is gradually released, having time to exert its effect. But if you introduce it in some other way, deeply and without papules, it will immediately “fall through” into the general bloodstream and will not work. Being in these small peas, the “hyaluronic acid” penetrates into the dermis in doses, attracts water, and a quick moisturizing effect occurs. The processes of collagenogenesis and improvement of cell function begin somewhat later. The maximum result after biorevitalization is achieved after 4-5 weeks.
— An interesting drug has appeared on the market - Pro fhilo. Tell us how it differs from other biorevitalizants?
— The main difference is that it does not leave behind those very “pimples” that we just talked about! The drug contains combined (stabilized and unstabilized) hyaluronic acid, it is injected very deeply and has a prolonged effect. Here the emphasis is not on hydration, but on collagenogenesis. In addition, we inject Profhilo at 10 bioaesthetic points, which correspond to the location of fat packets on the face. It's no secret that with age, fat packets shift downwards, which is why the face looks haggard. By placing the drug in certain areas, we return the lost volume to the tissues. Working at the level of deep layers allows us to combat pronounced age-related changes.
— Patients who are prone to swelling on the face are afraid to undergo biorevitalization. To what extent is their fear justified?
— With a properly selected drug, there should be no problems in principle. If there is swelling in the periorbital area, then drugs based on unstabilized hyaluronic acid should be injected here with extreme caution. In any case, if you are prone to edema, you should give preference to less hydrophilic drugs, for example, bioremedies.
— By what principle does a doctor select a drug for biorevitalization for a patient?
— The drugs differ in their molecular weight, concentration and the presence of auxiliary components. The main rule of choice: the more pronounced age-related changes are on the skin, the more intense medications will be required. For young patients under 30 years old, we take low concentrations of hyaluronic acid with the addition of vitamins, minerals, and amino acids. By introducing this “cocktail”, we actively moisturize the skin and nourish it with useful substances. And after 30-35 years, we use bioremedies - drugs with the addition of peptides, signaling molecules that have a receptor effect, forcing our cells to work as at a young age. Sometimes they contain vitamin C, one of the most powerful antioxidants.
— Which biorevitalizants have the leading positions on the market?
— There are a lot of worthy biorevitalizants. I would highlight the preparations from the French manufacturer Filorga and the Italian manufacturer Viscoderm Skinko E - they contain everything you need to build cells: minerals, vitamins, amino acids. With Filorga we work using the papular technique; it is more suitable for young skin. And Skinko E is administered at 19 bioaesthetic points. There are monopreparations containing partially stabilized hyaluronic acid. For example, Belotero Soft, which is used to reinforce leather. It does not retain volume in the tissues; it is injected as a biorevitalizant. Belotero Soft is especially good for the neck area. Juvederm Volite and Restylane Vital are preparations for prolonged biorevitalization, they also contain partially synthesized hyaluronic acid. Their introduction has its own characteristics; they are introduced very deeply. Biorevitalizants Meso-Wharton and Aquashine are very similar to each other, they contain more than 50 components, we often recommend them to young girls who are concerned about dry skin and dull complexion. There is a Russian-made drug, Hyalrepair, which contains only five components: hyaluronic acid, vitamin C and three amino acids. It works great for skin quality.
— Who is categorically contraindicated for biorevitalization?
— The procedure cannot be performed during pregnancy and lactation, with inflammation on the face, exacerbations of acne, herpes, psoriasis. People with very sensitive skin and severe rosacea should also approach biorevitalization with caution. Because there is a risk of new blood vessels appearing. Another contraindication is autoimmune diseases such as scleroderma and lupus erythematosus.
— How effective are cosmetics and dietary supplements with hyaluronic acid?
— As for cosmetics, in my opinion, this is nothing more than a marketing ploy. Hyaluronic acid is not able to penetrate the skin superficially. In creams, it only creates a thin film on the surface of the skin. People with dry skin often complain that creams and serums with hyaluronic acid do not help them. This is completely natural because they need lipids (molecules that create a protective barrier on the skin). As we age, we all lack lipids, and special cosmetics containing ceramides help to at least partially replenish them.
Taking dietary supplements is also almost not justified. Unchanged hyaluronic acid from the intestines cannot enter the skin. Because it is already being built on site. And instead of supplements, I would recommend paying more attention to proper nutrition. For healthy and elastic skin you need a nutritious diet, quitting smoking, tanning, proper home care, healthy sleep and treatments from a cosmetologist!
